Inspection 50-333/76-21

This refers to your letter dated November 20, 1978, in response to our

letter dated October 30, 1978, and to a discussion between the inspector

and Mr. Leonard on site on December 7, 1978, at the conclusion of Inspec-

tion No. 50-333/78-28.

Thank you for informing us of the corrective and preventive actions

documented in your letter.

These actions will be examined during a

subsequent inspection of your licensed program.

Our review of the additional information cn item B indicated that the

controls specified by TS 6.11 (A) 2 were required in that equipment was

accessible in this area and during inspection and servicing the radiation

dose rate to the whole body could exceed 1,000 mrem /hr. Specifically,

your survey record dated September 26, 1978, identified a 1,000 mrem /hr

gamma radiation dose rate at the entrance to the area of the drywell

sump. Subsequent surveys identified up to 20,000 mrem /hr gamma radiation

dose rate at 3 inches from a valve, and several thousand mrad /hr beta

radiation dose rate in thie vicinity.
