APPENDIX A NOTICE'0F VIOLATION Combustion Engineering, Incorporated Docket No. 70-1100 Based on the :esult of an NRC inspection conducted on October 24-27, 1978, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC License No. SNM-1067 as indicated below.
This first item is an infraction. The second item is a deficiency.
1.
Amendment No.17 to your license dated May 25, 1978, incorporates Section 8.2 " Nuclear Fuel Manufacturing - Windsor," Page VIII - 6, dated March 15, 1978, into your license.
Section 8.2 requires that, " Signs, including criticality limits as approved by the Nuclear Licensing and Safety Supervisor are posted near the appropriate work station."
Contrary to the above, on October 25, 1978, sit.s including criticality limits were not posted at the work station for the loadi_ng of fuel bundles into shipping containers and the storage location for calibration standard 55 gallon drums and waste beari_ng 55 gallon drums awaiting nondestructive assay.

10 CFR 71.12(b)(1)(i) and (ii), General license for shipment in DOT specification ~ container, requires that:
(i) the licensee have a copy of all documents referred to in the certificate of compliance for the shipping container; (ii) the licensee comply with the terms and conditions of the certificate of compliance for the shipping container.
Contrary to the above, on October 26,1978, :(i) the licensee did not have copies of Applied Design Company Drawing no. 927Al (modified)
Sheets 1, 2, and 3 referred to in item 5 (a)(3) of Certificate of Compliance No. 6078 for the Models Numbers 927Al and 927C1 fuel bundle shipping :antainers; (ii) the licensee failed to comply with condition 5(a)(3) of the certificate of compliance in that the Model 927Al container was not constructed in accordance with Supplied Design Company Drawing No. 927A1, Sheets 1, 2, and 3 as revised by Applied Design Company Drawing No. 927Al (modified) Sheets 1, 2, and 3.
The upper bundle bracket was secured to the strong back with eight 3/4-16 UNF hex head bolts rather than with ten 3/4-16 UNF hex head bolts with associated washers and nuts called for in the drawings.
