flARRATIVE REPORT On 1-19-80 the ?30KV / 4160V startup transformer 2D was taken out of service at 0430 hours0.00498 days <br />0.119 hours <br />7.109788e-4 weeks <br />1.63615e-4 months <br /> for testing vurposes.

Personnel misinterpreted Technical Specifications 3.8.1.1.a which requires two physically independent circuits between the offsite transmission network and che onsite class lE Distribution System be operable.

Since there were 2 500 Ki lines and 4 230 KV lines coming into the switchyard, personnel felt at this time that the requirement of Technical Specification 3.8.1.la was being met.

However, after reviewing technical specifications and the final safety analysis report, personnel realized at 0620 hours0.00718 days <br />0.172 hours <br />0.00103 weeks <br />2.3591e-4 months <br /> that there was not 2 independent circuits available to the Class lE Distribution System and that the requirements of Technical Specifications 3.8.1.1.a was not being r :!t with the startup transformer 2D out of service. Therefore two hours had elapsed since startup transformer 2D was taken out of service before perscnnel realized that there was only one physically independent circuit available to the Class lE l

Distribution System. Technical Specification 3.8.1.1 Action a requires that within one hour after the offsite power sources to the Class lE Distribution System is reduced to only one, that correct breaker alignment of the Class lE Distribution System be verified and that the diesel generators start from am-bient conditions and accelerates to synchronous speed in < 12 seconds.

hours bad elapsed from the time the 2D startup transformer was taken out of service before personnel realized the offsite power supply to the Class lE

- Distribution System was reduced to only one supply.

Therefore the requirement of Technical Specifications 3.8.1.la. was not met.

Procedures were ictediately started verifying correct breaker alignment and diesel generator starting re-l quirements.
