APPENDIX A NOTICE OF VIOLATION Based on the results of the NRC:I inspection conducted on February 22-25, 1977, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC License Nos. DPR-50 and CPPR-66 as indicated below.
These items are Infractions.
A.
Unic 1:
1 10 CFR 50, Appendix B, Criterion XVI states in part, " Measures shall be established to assure that conditions adverse to quality.
.. are promptly identified and corrected."
The accepted Goera-tional Quality Assurance Plan (FSAR Appendix 1A) Section 6.7 states in part:
"The Station Superintendent is directly responsible for.
.. insuring that conditions adverse to quality, when identified, are corrected for all activities involving operations, maintenance, repair, refueling, testing, and site engineering."
Contrary to the above, the corrective action delineated in Metro-n 1

politan Edison Company response letters to NRC:I Enforcement Cor-respondence dated May 28,1976 (Inspection No. 76-11) and August 27,1976 (Inspection No. 76-19) had not been completed as of
_j February 25, 1977. Specifically, for Inspection No. 76-11 adminis-teative procedures were to specify the review frequency for opera-tional procedures and the mechanisms to insure these reviews were to be in effect by November 24, 1976, and for Inspection No. 76-19 all Maintenance Department personnel whose work could involve lifting leads or placing jumpers were to be trained or retrained in the requirements and responsibilities imposed by Administrative Procedure 1013 by November 1, 1976.
B.
Unit 2:

The accepted Quality Assurance Program for Startup and Testing (FSAR Section 17.3),
Section 17.3.2, item 3 states in part:

Contrary to the above, measures had not been established and docu-mented nor had conditions adverse to quality been corrected as soon as practicable in that:
Finding 4 of Audit 76-12 dealing with an inadequate qualifica-tion procedure and a lack of certification of personnel per-forming safety related inspections was identified in a letter dated July 13, 1976 and was neither answered nor corrected as of February 24, 1977; and, Finding 8 of Audit 76-12 dealing with a failure to define, in writing, the authorities and responsibilities of certain per-sonnel performing activities affecting safety was identified in a letter dated July 13, 1976 and was neither answered nor corrected as of February 24, 1977.
These two findings represent a failure to answer / correct approxi-mately 10% of the audit findings conducted under the Startup and
