Reportable Occurrence No. 50-366-1979-29, Rev. 1

'I Event Description and Probable Consequences were not aff ected by this incident.

Cause Description and Corrective Actions bushin;;s were manuf actured frcu ASME SA564 type 630 (17-4PH) stainless steel instead of the criginal ASME SA479 type stainless to deter any future bending. The valves were then leak tested and returned to the plant site.

All valves have been inspected and reinstalled on the appropriate relief valve discharge lines.
