APPENDIX A NOTICE OF VIOLATION Power Authority of the State of New York Docket No. 50-333 Based on the results of the NRC inspection conducted on November 13-17, 1978, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-59 as indicated below. This item is an Infraction.

Technical Specification 6.11 states in part:

" Procedures for per-sonnel radiation protection shall be prepared and adhered to for all plant operations".

Radiation Protection Operating ProceduresSection III, "Contami-nation Control", specifies in A.5, " Roping and Tagging" and G.1,

" Contamination Guides - Area Contamination", that barricades shall be utilized to delineate contaminated areas in the station in excess of 400 d/m per 100 cm4 as determined by a disc smear and that tags shall be placed on these barricades to indicate condi-tions within the barricaded area.

Contrary to the above, at approximately 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> en November 16, 1978, the main turbine area located between the north and south shield walls on the 300 foot elevation of the Turbine Building (turbine deck), maarea identified to have contamination levels of up to 1860 d/m per 100 cm2, was not barricaded (the east and west doors were blocked open) nor tagged to indicate the contaminated area.
