Dear Mr. Grier:
With reference to the inspection conducted by Mr. T. Stetka of your office on November 13-17, 1978. at the James A. Fit: Patrick Nuclear Pcwer Plant, and in accordance "ith the provisions of Section 2.201 of Part II of Title 10 of the Code of Federal Regulations we are submitting our reply to Apperdix A Notice of Violation transmitted by your letter dated December 7, 1978 as received by the undersigned on December 11, 1978.
APPENDIX A Notice of Violation Based on the results of the NRC inspection conducted on November 13-17, 1978, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. DPR-59 as indicated below. This item is an Infraction.
Technical Specification 6.11 states in part:
" Procedures for personnel radiation protection shall be prepared and adhered to for all plant operations".
Radiation Protection Operating Pi.-uduresSection III, " Contamination Control", specifies in A.5, " Roping and Tagging" and G.1, "Contamina-tion Guides - Area Contamination", that barricades shall be utilized to delineate contaminated areas in the station in excess of 400 d/m 2
per 100 cm as determined by a disc smear and that tags shall be placed on these barricades to indicate conditions within the barricaded area.

Page 2 JAFP Contrary to the above, at approximately 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on November 16, 1978, the main turbine area located between the north and south shield walls on the 300 foot elevation of the Turbine Building (turbine deck), an area identified to have contamination Icvels of 2
up to 1860 d/m per 100 cm, was not barricaded (the east and west doors were blocked open) nor tagged to ir.dicate the contaminated area.
Response to Appendix A This incident occurred during the clean up of the main turbine area after overhaul.
Ropes and tags had been placed at the north end of the area but had been mistakenly set aside by the clean up crew to facilitate removal of tools and cleaning equipment to an area prepared for decontam-ination of these items.
Upon notification of this condition radiation protection personnel replaced the ropes and tags. The clean up crew working in the area was instructed that they were not to move any barricades, signs, ropes or tags in the area.
The radiation protection technician assigned to the area was instructed to increase his surveillance of the area to ensure that proper procedures were followed by personnel in the area in order to prevent recurrence.
Compliance with our procedures was achieved on the day of the incident.
