YELLOW CREEK NUCLEAR PLANT UNITS 1 AND 2 FAILURE TO PERFORM PROPER CHEMICAL ANALYSIS ON SUBMERGED ARC WELDING MATERIALS INFRACTION 50-566/50-567/79-17-01 Infraction 79-17-01 As required by paragraph (a)(1) of 10 CFR 50.55(a), Structures,. ..

shall be. . . constructed, tested, and inspected to quality standards commensurate with the importance of the safety function. . . For the containment structure, this requirement is implemented by paragrapns 3.8.2.2 and 3.3.2.6 of the PSAR which require that all material meet Article NE-2000 of Section III of the ASME Code. Paragraph NE-2342.l(b) of the applicable addenda of NE-2000 states that ". . .other welding material. . .shall have chemical analysis performed on a weld deposit of. . .the combination of materials being certified."

Contrary to the above, on November 7, 1979, according to materials certification records, a weld deposit chemistry was not made for submerged arc welding materials EH14 (HT.036057) and 860 Flux (Lot EH).

Corrective Action Taken and Results Achieved Chicago Bridge and Iron Company (CBI) is the vendor for the Yellow Creek steel containment structures. Upon being notified of the lack of weld chemical documentation, CBI initiated a testing program which provided the necessary chemical analysis information for all CBI welds on the containment. The chemical analysis demonstrated that all weld material is in compliance with Code requirements. CBI Certificates of Test have been revised to include the chemical analysis information. CBI quality assurance employees at the site have entered these revised Certificates of Test in the site records.

Steps Taken to Avoid Recurrence To prevent a recurrence of this omission, CBI has revised and reissued the Weld Material Test Procedures WMT-30 and WMT-31 to include reporting the chemical analysis for containment veld materials on the Certificate of Test.

Date of Full Compliance The revised Certificates of Test were entered in the site records on December 7, 1979. The Weld Fbterial Test Procedures were revised and issued on December 12, 1979. The revised Weld Material Test Procedures were received onsite and entered into CBI procedure book on December 19, 1979. Therefore, Yellow Creek Nuclear Plant was in compliance on December 19, 1979.
