This letter refers to the telephone conversation between you and Mr. R. T.

Carlson of this office on November 7,1979 concerning the omission of reinforcing steel in the containment building crane wall of Beaver Valley Pow.r Station 'Jnit 2 as reported by Duquesne Light Company in accordance with 10 CFR 50.55(e) on November 2,1979.

The specific area of the crane wall from which reinforcing steel was omitted is that section extending from elevation 757'-7" to elevation 762'-5" and which encompasses the azimuthal area from approximately 107 to 134*.

Based on the above conversation, it is our understanding that you have accomplished, or plan to accomplish, the following:

1.

Issue a stop work order to halt placement of safety related concrete and preclude further placements until all of the actions identified below have been accomplished.

2.

Prepare a detailed construction procedure for the removal of the deficient section of the crane wall.

3.

Conduct quality control activities during and after the removal of the deficient crane wall section to assure interfacing wall sections and structural components are not adversely affected.

4.

Perform a review of drawings and concrete placement records related to previous crane wall placements to determine the extent of the problem.

5.

Revise the inspection procedures as appropriate to require inspectors to utilize both the rebar detail drawings and the general engineering drawings when conducting preplacement inspections of rebar ir.stallations.

6.

Provide additional training for quality control inspectors in relation to utilization of the revised procedures.

7.

Conduct an audit of the Stone & Webster design verification and drawing checking system to determine the cause of the omission of the rebar from the detail drawing, and implement corrective action.
