' injection pumps is via a path controlled by parallel motor-operated isolation valves which are normally closed. At 0637 hours0.00737 days <br />0.177 hours <br />0.00105 weeks <br />2.423785e-4 months <br /> on November 27, the No. 1 Emergency Diesel Generator was removed from service for scheduled preventive maintenance. The generator provides emergency power to [CH-ll5B], one of the two parallel motor-operated isolation valves on the RWST. At about 0830 hours0.00961 days <br />0.231 hours <br />0.00137 weeks <br />3.15815e-4 months <br />, the redundant motor-operated isolation valve [CH-ll5D] was placed on clearance, in the closed position, to correct a motor-operator malfunction.

The flow path to the charging pumps would have been available with offsite power available. In the event of a simultaneous requirement for high head safety injection and a loss of of fsite power, high head injection would only have been available until the Volume Control Tank emptied. The incident was caused when clearance approval was granted in the midst of normal early morning demands on operator time.

The error was discovered by the licensed operator upon reflection on earlier morning activities at 1015 hours0.0117 days <br />0.282 hours <br />0.00168 weeks <br />3.862075e-4 months <br />. The valve was opened at 1029 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.915345e-4 months <br /> providing the required flow path to the safety injection pumps.

The Onsite Safety Committee has reviewed the incident and has determined to adopt and implement administrative procedures to perform redundant independent verificatirn of the opetrbility of the remaining ESF equipment whenever any safety system or subpart is intenti'onally removed from service.

A detailed review of existing proceduras and controls shall be performed to assure that the limiting conditions for operation are not defeated by maintenance or other activities.
