when secondary containment integrity was not maintained when the personnel access airlock doors were found open.
This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2.a.6.

DESCRIPTION On Monday, October 8, 1979, at approximately 1630 hcurs, an 0F OCCURRENCE:
operator discovered both personnel access airlock doors in the northeast corner of the Reactor Building (door #19) were open simultaneously.
Since this was a breach of secondary containment integrity, both doors were immediately closed.
The operator then reported the occurrence to the Group Shift Supervisor.
The apparent cause of this condition was caused by contractors using this passage.

ANALYSIS OF Secondary containment is required to minimize ground level release OCCURRENCE:
of airborne radioactivity materials, and to provide for controlled, elevated release of the building atmosphere under accident conditions.
The ability of secondary containment to perform its function with both airlock doors open was degraded.
