Dear Mr. Lewis:

This refers to the opparent infraction it the subject inspection report which states:

As required by 10 CFR 50 Appendix B, Criterion XI Test Control whfch says in part "A test program shall be established to assure that all testing required to demonstrate the structures, systems, and components will perform satisfactorily in service is identi-fled and performed in accordance with writt en test procedures..."

  • These requirements are incorporated in Alab una Power Company Opera-tions Quality Assurance Manual, Chapter 11, paragraph 11.4.1 which says in part, "The test program is to be identified, documented, scheduled, and accomplished in accordance with controlled proce-dures", and implemented by Alabama Power Company's Joseph M. Farley Nuclear Plant Startup Standard 6, paragraph 5.6.4 which says in part, "The Official Test copy of the procedure, including all approved modifications, will be followed in performing the test".

Contrary to the above, during the performance of Test Procedure C24-5-002, Diesel Generator 1-2A, 1C and 2C, tn Unit 2 Pre-op, a jumper was installed in a Unit 1 diesel generator breaker control cir;.vit vice the specified Unit 2 diesel generator breaker control circuit. At the completion a jumper installed by the procedure was not removed as specified in the procedure.

The psent Startup Croup has taken the following corrective action:

A. The first jumper mentioned in the subject report was correctly re-installed. The second jumper was subsequently removed.

!- Mr. R. C. Lewis Page Two June 19, 1979 B. A training session was held with Startup Test Engineers on Startup Standard 6, " Test Procedure Development, Conduct, and Results Approval". Emphasis was placed on sections 4.6 and 5.6 (Test Conduct). This training was completed on 3-21-79. Also, this training has been documented.

It should be noted that although the subject report was a Unit 2 re-port, the findings were discussed at the exit conference with the Production Depar tment. Neither the Manager Nuclear Generation nor the Plant Manager were on distribution for the report; therefore, the tracking mechanism (logging the infraction and its corresponding response date) used by the General Office and Document Control to assure a timely submittal of the response, was not implemented. It is requested that H. O. Thrash, Manager Nuclear Generation, 600 North 18th Street, Birmingham, AL 35203; and W. G. Hairston, III, Plant Manager, P. O. Drawer 470, Ashford, AL 36312, be added to the distribution of inspection reports relating to Unit 2 of Farley Nuclear Plant.

We do not consider the information in the subject report to be of a proprietary nature.
