APENDIX A NOTICE OF VIOLATION Vennont Yankee Nuclear Power Corporation Docket No. 50-271 Based on the results of an NRC inspection conducted on October 3-6, 1978, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Facility License No. DPR-28, as indicated below.
These items are Infractions.
A.
Technical Specification 6.5.B.1 states in part: "In lieu of the
' control device' or ' alarm signal' required by paragraph 20.203(c)
(2), each high radiation area in which the intensity of radiation is 1000 mrem /hr or less shall be barricaded and conspicuously posted as a high radiation area...."
Contrary to the above, at approximately 2400 hours0.0278 days <br />0.667 hours <br />0.00397 weeks <br />9.132e-4 months <br /> on October 3,1978, the Control Rod Drive repair room and Control Rod Drive laydown area, posted as high radiation areas, were not barricaded.
B.
Technical Specification 6.5.A states in part: " Detailed written pro-cedures... covering areas listed below shall be prepared and approved.
All procedures shall be adhered to."
Refueling operations is an area covered by the above specification.
Procedure OP 1101, Revision 7, Fuel Assembly Movement, states that Source Range Monitors (SRM) must be continuously monitored whenever fuel is being moved in the Reactor Vessel.
Contrary to the above, on October 4,1978, fuel bundles were inserted into the Reactor Vessel while the SRM's were not being continuously monitored.
