APPENDIX A NOTICE OF VIOLATION Mississippi Power and Light Company License No. CPPR-118 Grand Gulf Nuclear Power Station Based on the NRC inspection August 21 through September 7, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspon-dence to you dated December 31, 1974.
As required by Criterion V of Appendix B to 10 CFR 50 " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures, or drawings."
Bechtel P&I Diagram No. M-1087 Rev. 10 and Piping System Isometric Drawings
- M-1350A Rev. 17 specify that low pressure core spray system motor operated globe valve F012 be installed with the packing gland on the upstream side of the valve disc.
Contrary to the above, in Unit 1 valve F012 is installed with the packing gland on the downstream side of the valve disc.
This condition has been verified by information radiographs.
The valve inspection record for valve F012 has been signed off by QC Engineering as having correct flow orien-tation.
This is an infraction.
