A.4.a, when a reactor building to torus vacuum breaker did not open during a survelliance test.
This event is considered to be a reportable occurrence as defined in the Techni-cal daecifications, paragraph 6.9.2.h.2.
Condicions Prior to Occurrence The plant was operating at steady state power.

Description of Occurrence On Friday, December 28, 1979, at approximately 3:00 p.m., while performing the reactor building to suppression chamber air operated vacuum breaker test, valve V-26-16 failed to operate.
Additional open signals were applied to V-26-16 and after approximately 21/2 minutes, the valve opened.
Subsequent cycling of V-26-16 failed to repeat the original problem, but there was evidence of binding.
This "alve was cycled daily from the date of occurrence to the outage January 6, 1980.

Reportable Occurrence Page 2 Report No. 50-219/79-43/3L Analysis of Occurrence The purpose of the vacuum relief valves is to equalize the pressure between the suppression chamber and the reactor building so that the containment external design pressure limits are not exceeded.
The vacuum relief system from the reactor building to the suppression chamber :onsists of two 100% vacuum relief breaker subsystems. The vacu'sn breakers are sized in relation to the vent from the chamber so that make-up flow under any conditions following venting is ade-quate to prevent external pressure greater.han one half the design pressure.
With the vacuum relief system in the degrased condition, the syst-m was still capable of venting the suppression chamber if it had been necessar '.
Corrective Action
A Job order has been initiated to inspect the valve this outage. The valve will be tested following maintenance. A PORC Action item has been issued to report back to the PORC the results of the maintenance investigation on the valve failure mechanism.
