REPORTIf;G SIGilIFICANT SAFETY IllFORMATI0f1 Attached are a collection of memoranda relating to a recent lapse in the reporting of significant safety information.
I wish to emphasize again how important it is to report promptly any information which you receive that may have significance to safety and which you have reason to believe has not been reported to an appropriate Office Director or higher level of flRC management.
Such information-should be reported promptly and directly to your Division Director, who, in turn, is responsible for pr o. aptly transmitting the infomation to the appropriate flRC official (s) for action and necessary further transmittals.
If there is any doubt in your mind as to whether the information has been previously reported, you should report the information to your Division Di rec tor.
As the Executive Director for Operations stressed in his August 21, 1979 memorandum (attached), for IIRC employees there is an
" absolute, unfailing requirement that any matter of safety significance be promptly reported".
