Beaver Valley Power Station - Unit No. 2 Reactor Containment Liner - Overlay Pad Fillet Weld Crack Docket No. 50-412 Significant Deficiency Report No. 78-03 Gentlemen:
Pursuant to the requirements of 10CFR50.55 (e), Duquesne Light submitted a final written report on the subject problem March 22, 1979.
Your review of that submittal has resulted in our revision of the final report to delete both the Proprietary Notice and Disclaimer of Responsibility from of the report.
We believe that these revisions satisfy your earlier concerns and, therefore, herewith submit the revised final report.
If you have further questions regarding our submittal, we can meet with the appropriate Nuclear Regulatory Commission technical personnel at their convenience.

Beaver Valley Power Station - Unit No. 2 Reactor Containment Liner - Overlay Pad Fillet Weld Crack Docket No. 50-412 Significant Deficiency Report No. 78-03 Gentlemen:
Field attachment of an overlay pad to the containment liner cylinder, has resulted in the cracking of the wall liner plate.
The crack was found to exist along the hori-zontal, bottom side of the overlay pad, adjacent to the continuous fillet weld used to make the attachment.
The problem was reported to Mr. Seth Folsom of your office, on December 22, 1978, as a significant deficiency.
An investigation into the cause of the cracking is currently underway.
Pursuant to the requirements of 10 CFR 50.55(e), a written interim report is herewith sub-mitted, and it is presently anticipated that a final written report on this matter will be submitted to you on March 22, 1979.
