Certain of your activities under your license appear to be in violation of AEC requirements.
These violations are considered to be of Category II severity:
1.
Technical Specifications require in Table 4.1-2, Item 7, that Reactor Coolant System Leakage be evaluated daily when reactor coolant system temperature is greater than 5250F.
Contrary to the above, there was no evidence furnished to indicate that the required evaluation had been performed on either May 19 or May 20 1974 while the reactor coolant system was greater than 5250F.

Section 13.1.1 of the FSAR for Three Mile Island Unit 1 states, in part, "The test program shall be conducted in a planned and systematic fashion and in accordance with procedutes and policies contained in the Three Mile. Island Nuclear Generating Station Test Manual and individual Test Instructions."
Specifically Test Instruction 9 (TI-9), Conduct of Test, Paragraph 3.3.
2.6 titled Changes and Exceptions to Test Procedures states, in part,
"...e. Test Change Notice (TCN) is required if the proposed change affects the procedure's scope or intent."
Contrary to the above, during control rod testing on May 22, 1974, a change to a test procedure (TP 330/5 CRD TRIP TEST) was made which changed the scope of the procedure without the required TCN.
