PRELIMINARY NOTIFICATION June 7, 1979 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-138 This preliminary notification constitutes EARLY notice of event of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Crack in Feedwater to Steam Generator Weld As a result of a letter sent.to all PWR licensees from the Division of Operating Reactors /NRR dated May 25, 1979, radiographic examinations were performed on the "B" feedwater to steam generator nozzle welds on June 4,1979.
The results of these examinations show a circumferential crack in the "B" feedwater nozzle to the pipe reducer weld.
The crack appears to have started at the root pass of the weld, has propagated from the inside toward the outside and has a depth about 50% of the thickness of the pipe weld.
Ultrasonic tests are being conducted to further evaluate the radiographic indications.
The "A" and "C" feedwater to steam generator nozzles will also be examined.
Region V Reactor Inspectors will be onsite commencing June 6, 1979 to review and evaluate this occurrence.
The facility is currently in the cold shutdown condition for a maintenance outage that started June 2,1979.
As a result of this feedwater to steam generator nozzle weld crack occurrence, the maintenance outage is expected to be extended for an undetermined period.
News media interest is expected. The licensee plans to issue a news release.
A news release by NRC is not planned.
Region V (San Francisco) received telephone notification of this occurrence from the Plant Engineer at 9:30 A.M. on June 5, 1979, with additional informa-tion 9rovided by telephone at 1:15 P.M. on June 5, 1979.
