PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-79-191 This preliminary notification constitutes EARLY notice of events of POSSIBLE b
safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subject:
INADVERTENT ACTUATION OF CONTAINMENT SPRAY At approximately 4:00 a.m.,
on June 25, 1979, while performing surveillance testing in preparation for startup, the upper (above ice condenser) contain-ment area was sprayed with approximately 1,000 gallons of primary system water when a pressurized containment spray line valve was cycled open for periodic testing.
The east RHR pump was operating and was cross tied to the west RHR heat exchanger.
While this is not a standard lineup, it is acceptable. The valve cycling surveillance testing, however, is keyed to whichever RHR pump is operating and does not address the cross tie situation. Thus, with the east RHR pump operating, the test calls for the west containment spray valve to by cycled.
In this case, since the east RHR pump was cross tied to the west RHR heat exchanger, the west containment spray valve was pressurized.
The error was recognized immediately, and the valve was closed af ter being open for an estimated 15 seconds.
The airborne contamination levels inside containment reached 10 exponent -8 microcuries/ cubic centimeter.
There were no releases to the environs.
Cleanup is in progress.
The resident inspector is onsite.
The State of Michigan has been notified by the licensee.
Both Region III and the licensee will issue press releases.
The Region III inspector received notification of the occurrence at approximately 4:10 a.m, on June 25, 1979, by telephone from the licensee.
