This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2.b.2.

Conditions Prior to Occurrence The major plant parameters at the time of the occurrence were:

Power: Reactor,1929 MWt Electrical, 672 MWe Flow: Recirculation, 15.8 x 10 4gpm Feedwater, 7 2 x 10 6 lb/hr Pressure: 1020 psig Stack Gas: 3.49 x 104 pci/sec Description of Occurrence On Tuesday, December 12, 1978, Standby Gas Treatment System 11 discharge valve V-28-30 failed to close following the performance of the routine system survelliance test. The valve was subsequently cycled by hand, and in follow-

~ lng operations, opened and esosed properly. At that time, maintenance per-sonnel checked the ope ator solenoid as it had been a problem in past failures.

The solenoid was in good working order and the valve was operating, though with some evidence of friction. Following the performance of the next scheduled sy, tem surveillance test on Tuesday, December 19, 1978, System 11 discharge valve V-28-30 again failed to close following the test. Performance of the valve was similar with no failures during subsequent operations.

Maintenance personnel inspected the valve operator and lubricated the piston cylinder. Again, the valve operated with no failures following the mainte-nance. On Tuesday, December 26, 1978, during performance of the scheduled system surveillance test, valve V-28-30 failed to open on the first attempt Jersey Central Power & Light Cornwny is a Member of tne General Public Utilities System

. r Reportable Occurrence No. 50-219/78-36/3L-0 Page 2 January 10, 1979 to initiate System II. On subsequent system operations, the valve cycled properly each time. Maintenance personnel have replaced the valve operator.

No further failures have been experienced.

Apparent Cause of Occurrence The apparent cause of the incident was the buildup of dirt and corrosion in the piston cylinder of the valve operator.

Analysis of Occurrence The Standby Gas Treatment System is designed to minimize the release of radio-active material to the environs during secondary containment isolation conditions by filtering the reactor building atmosphere prior to exhaust to the stack. Each of the two separate filter systems provide 100% capacity.

If one filter train becomes inoperable, there is no immediate threat to secondary containment. At the time of each occurrence, Standby Gas Treatment System I was demonstrated operable. Had conditions existed which required operation of the Standby Gas Treatment System, System I wou* ' have performed the required function.

Corrective Action The valve operator for Standby Gas Treatment System 11 discharge valve V-28-30 was replaced.
