Dear Sister Grimes:

This refers to the inspection conducted by Mr. T. M. Tongue of this office on August 27 and 28, 1979, of activities at St. Mary's Hospital authorized by NRC Byproduct Material Licenses No. 48-01828-01 and No. 48-01828-02; and Special Nuclear Material License No. SNM-1804 and to the discussion of our findings with Dr. Ruetz and Mr. Roberts at the conclusion of the inspection.

The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.

With respect to License No. 48-01828-01 during this inspection certain of your activities appeared to be in noncompliance with NRC requirements, as described in the enclosed Appendix A.

With respect to Licenses No. 48-01828-02 and No. SNM-1804 no items of non-compliance with NRC requirements were identified during the course of this inspection.

The inspector also examined actions you have taken with respect to the matters identified in your letter dated May 11, 1977. We are concerned about your actions with respect to records of source inventories and leak tests. During the last inspection a citation resulted from failure to maintain records in these areas. During this inspection your records were found to be extremely disorganized and so poorly documented that they could only be understood or interperted through, or explained by your Radiation Safety Officer, Dr. Chan. We are concerned that in the absence of Dr. Chan interpertation and location of these records would be imposs-ible.

In your response to this letter please state what actions you will take on this matter. We have no further questions on other statements in your letter dated May 11, 1977.

Section 2.201 requires you to submit to this office within twenty days of your receipt of this notice a written statement or explan-

. ation in reply, including for each item of noncompliance: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.

We will gladly discuss any questions you have concerning this inspection.
