Appendix A NOTICE OF VIOLATION St. Mary's Hospital License No. 48-01828-01 Based on the inspection conducted on August 27 and 28, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Item 1 is an infraction, and item 2 is a deficiency.
1.
Condition 19 of your license requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated April 26, 1979 and letter dated June 2, 1979. Your application dated April 26, 1979, Item 14.D. states in part that all packages received by the Depart-ment of Nuclear Medicine are to be checked for contamination.
Contrary to the above, your records show that packages containing byproduct material, received by the Nuclear Medicine Department have not been surveyed for radioactive contamination from June 18, 1979 to the date of this inspection.

10 CFR 30.51(a) requires that records be maintained showing receipt, transfer, and disposal of byproduct material.
Contrary to the above, your records of transfer of byproduct material are incomplete. Specifically you have no records showing the transfer of used vials and syringes back to your supplier.
