NRC Inspection This refers to the inspection conducted by fir. Paul Zurakowski of this office on April 23, 1979, of activities related to expired NRC License No. Sf'M-1425 and to the discussion of our findings held by Mr. Zurakowski with Dr. Shabetai and other members of your staff at the conclusion of the inspection.
The inspection confirmed that your sflM-1425 license had expired and that a timely renewal application had not been submitted. The inspection also confirned that a license is needed because you do have one patient with an implanted nuclear powered pacemaker containing plutonium.
We also understand that you have taken steps to acquire a new license.
As was pointed out in my letter of March 23, 1979, so that we are informed of your actions in writing, a copy of the letter fonvarding the license renewal application to the licensing branch (but not the application) should be sent to this office. This letter is required in addition to Mr. Zurakowski's discussion and observations during tia inspection.
