APPENDIX A NOTICE OF VIOLATION Virginia Electric and Power Company License No. CPPR-114 North Anna, Unit 3 Based on the NRC inspection May 29 through June 1, 1979, c rtain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Criterion V of Appendix B to 10 CFR 50, implemented by Section 5 of the VEPCO Quality Assurance Manual, " Activities aff-cting quality shall be prescribed by documented instructions, procedures, or drawings,.... and shall be accomplished in accordance with their instructions, procedures or drawings". North Anna Specification NAS 30113 and reference NRMCA specification requires that cc:. crete batch plant scales and ceasuring devices be calibrated every 90 days.
Contrary to the above, the concrete batch plant scales were not calibrated between March 24, 1978 and April 28, 1979.
This is an infraction.
B.
As required by Criterion V of Appendix B to 10 CFR 50, implementeo by Section 5 of the VEPC0 Quality Assurance Manual, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings,....and shall be accomplished in accordance with these instructions, procedures or drawings".
Construction Quality Control Procedure 10.2, " Concrete Quality Control", requires that concrete be sampled in accordance with ASTM C-172.
ASTM C-172 requires that tests for slump and air content be started within 5 minutes after sampling of concrete is completed.
Contrary to the above, slump and air content tests conducted in the laboratory on samples of concrete exceeded the 5 minute limit for 4 of 10 samples tested on May 30, 1979.
This is c deficiency.
