APPENDIX A NOTICE OF VIOLATION Westinghouse Electric Corporation License No. SNM-1107 Based on the NRC inspection on May 8-11, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by License Condition No. 9 incorporating the license application subsection 3.1.2, " Position Responsibilities," states, in part, that a line manager will assure that all operations are carried out in accordance with the schedules, processing procedures, nuclear criticality safety and radiation protection limits, and other instructions supplied by higher management.
Contrary to the above, line management permitted operations to be performed which were not in accordance with the posted instructions for the storage of special nuclear materb ls (SNM) in the QC Lab storage cabinet.
The posted instructions require, among other things, that the storage shelves be separated by at least 16 inches.
In contrast, the shelves in the storage cabinet were separated by 15 inches.
This is a deficiency.
