SUMMARY

INFORMATION REPORT Week Ending July 6,1979 Pioe Cracks in THI-l The licensee (Metropolitan Edison Company) for Three Mile Island, Unit No. I has examined'15 weld joints of the decay heat removal system by visual and ultrasonic examination and has found cracks in four pipe welded joints,

.three of which are through wall cracks. The results of this examination indicates the cracks were due to high residual stresses. As of July 3,1979, the licensee inspected 457 weld joints, approximately 25% of which have some indication of defects. The severity of the defects has yet to be determined.

The NRC staff scheduled a site visit f6F July 5,1979.

Kewaunee As a result of an inspection conducted during the current refueling outage, cracks have been found in feedwater line welds at Kewaunee. The cracks are being repaired.

Point Beach Unit 2 As a result of an inspection conducted on July 2,1979, cracks have teen discovered in feedwater line welds at Point Beach Unit 2.

A meetirg was scheduled for July 6,1979 to discuss details of the repair program.

State Acreements Nevada Governor List ordered suspension of the license to NECO for operation of the low level rad waste burial site of Beatty.

The action was taken after a truck hauling radioactive waste from Consumers Power, Cobert, Michigan arrived at the site and the driver reported he believed the shipment was leaking.

When the shipment was off-loaded, two containers were found to be corroded and leaking radioactive liquid.

Action on NRC-USSR Cocoeration Proposal IP was advised on July 5 that the Executive Branch had cleared in its final form the proposed NRC-Soviet Ministry of Power and Electrification " Protocol for the Exchange of Nuclear Safety and Regulatory Information and Cooperation in Nuclear Safety Research in Light Water Reactors," and was air-pouching it to the U.S. Embassy in Moscow for hand-delivery.

and Part 95, " Security Facility Approval and Safeguarding of National Security Information and Restricted Data" were published for comment in the Federal Recister on Moaday, July 2,1979.

Part 25 contains procecures for establishing initial and con-tinuing eligibility of individuals for access to classified information received or ceve'. opec in conjunction with the licenses issued under Parts 50 or 70. Part 95 establishes procedures for the protection of such information at a facility.

Comments on these two parts must be received on or before August 31, 1979.

Classification Guide for Safecuards Information The Executive Director for Operations approved the NRC Security Classification Guide for Safeguards Information on June 29, 1979.

The guide will be furnished to NRC offices and NRC contractors as soon as prir. ting can te accomplished. A second classification guide, in draf t form oealing with the disposal of radioactive materials, will be suomitted to ~ offices for comment in the near future.

Point Beach Unit ?

As a result of an inspection conducted on July 2,1979, cracks have been discovered in feedwater line welds at Point Beach Unit 2.

The cracks exist in both ends of a 14" x 16" reducer which joins the feedwater piping to the steam generator nozzle. Repairs are anderway. A meeting is scheduled for July 6,1979 with the NRC staff in Bethesda to discuss the detafis of the repair program.

Brunswick Units Nos.1 and 2 CP&L informed the staff that two (2) supports on a Unit No. 2 HPCI/RHR j

OFFICE OF STANDARDS DEVELOPMENT IMPORTANT EVEbES FOR THE WEEK ENDING JULY 6,1979 1

The EPA Interagency Coenmittee on Federal Guidance for Occupational Exposure to Ionizing Radiation met on June 29, 1979, in an attempt to reach agreement on new EPA guidance to be published for connent in the Federal Register. This FR notice will also provide final details regarding the joint EPA / OSHA /NRC hearing to be held on this subject.

(The advance notice on the hearing is still in the EPA concurrence system.) The agency representatives were able to reach agreement on guidance to propose for the control of exposure from external sources. Briefly, the basic Radiation Protection Guide (RPG) would be 5 rems per year; any provisions for exceeding the RPG during planned special exposures, as well as any actions to be taken following overexposures, would be left to the discretion of the implementing regulatory agencies. A graduated scale of protective actions, dependent upon the degree of hazard present, would be recommended.

For personnel expected to receive less than 0.5 rem per year, few protective actions would be specified.

In the range of 0.5 to 1.5 rems per year, several additional actions would be listed.

For the r. ige 1.5 to 5.0 rems per year, the list of specified actions would be more extensive. No lifetime dose limit would be imposed, but the guidance would ccntain an exhortation to keep all individual, lifetime doses below.100 rems. There would also be a strong, unquantified, recommendation to control the collective dose.

The committee will meet on July 6,1979, to consider proposed guidance for the control of exposure from radionuclides deposited in the body.

Committee members were informed that the EPA will request formal

~

concurrence from each agency represented prior to publication of the FR notice.

Meeting with EPA on Clean Air Act:

On July 3 Robert Minogue (SD) met with Dr. David Rosenbaum, tt.e new EPA Deputy Assistant Administrator for Radiation Programs, to discuss implementation of the Clean Air Act for NRC licensees.

Others attend!ng were Enrico Conti and Jane Axelrad of NRC and Paul Magno and Tcdd Joseph o# EPA.

Since Dr. Resenbaum is new to EPA, Messrs. Minogue and Conti reviewed the Clean Air Act recuirements, including development of an EPA /NRC agreement covering NRC licensees.

They explained the different categorics of licensees (reactors, other fuel cycle, non-fuel cycle) and the problems associated with development and implementation of Clean Air Standards for each category. They emphasized NRC's goal f}l ON ENCLOSURE C

2-to avoid duplication of effort, especially for reactors where considerable effort has already been expended on implementation of Appendix I and 40 CFR 190.

We agreed with Dr. Rosenbaum to continue to provide information to EPA on our licensees, and he indicated that he would try to be as responsive to NRC as possible in implementing the Clean Air Act.

With regard to reactors, Dr. Rosenbaum suggested the possibility of setting a simple curie release limit which would be implerrented in addition to 40 CFR 190 and Appendix I, both of which use dose limits.

Up to this time, the EPA and NRC technical staffs have assemed that dose limits wot.!d be preferable as Clean Air Standards.

However, Dr. Rosenbaum and attorneys on both sides agree that curie or quantity release limits would seem closer to the intent of the Act. We agreed to look into the issue and report any potential problems with such release limits to EPA.

Week Endino July 5,1979 Commission Paper on Hich Assurance as an Objective A Commission Paper on using the term "high assurance" as an objective to replace use of the term " reasonable assurance" in the Safeguards Upgrade Rule has been prepared and forwarded for office concurrence. This action was taken in response to Chairman Hendrie's request that such changes be formalized based upon the Commission having reached a consensus on the changes. The same terminology would be adopted in the power reactor rule (573.55) as in the Upgrade Rule.

Oconee-McGui re Hearira A Safeguards represertative participated in the hearing during the period June 20 - June 29 in matters pertaining to postulates :abotage of spent fuel shipments. The r. earing was adjourned unti' August C at which time we expect to continue c >r participation.

Material Control & Accountino Procedures - Wilminoton. N.C.

Representative from the General Electric Company met with safeguards staff members on June 29, 1979, to discuss a current amendment request involving material control & accountability procedures at their Wilmington, North Carolina facility. On July 2 and 3, a safeguards licensing staff member visited the Wilmington facility to further discuss this and other matters related to material control & accountability at that facility.

Preliminary Notifications relating to the following actions were dispatched during the past week:

PNO-79-67AV Three Mile Island Unit 2 - Updated Status Report - This Preliminary Notification was issued to provide updated status information.

b.

PNO-79-204 Washington Nuclear Project No. 2 - Improper Installation of Concrete Anchor Bolts - The licensee discovered that six anchor bolts had been cut short and tack welded to support plates, giving the appear-ance of proper installation. The bolts provide anchorage for safety-related control room ventilation ducting. The licensee has issued a stop-work order to the contractor and has initiated a full investigation.

PNO-79-205 Millstone Unit 1 - Low Water Level Trip - On July 2,1979, the reactor tripped due to a loss of instrument air which caused the feedrater regulating valves to drift shut resulting in a low water level.

PNO-79-206 Rancho Seco - Automatic Plant Shutdown from Low Power Following Test of Manual Control of Auxiliary Feedwater - On July 1, 1979,

.the plant tripped from 15% power due to high reactor coolant pressure when a startup feedwater valve closed momentarily.

PN0-79-207 Davis-Besse Unit 1 - Timely Reporting of Infomation - During a surveillance calibration of pressure switches in the Auxiliary Feedwater System in May 1979, the lic.ensee discovered that six pressure switches were out of calibration. Although the licensee reported these results in a 30-day report as required by Technical Specifications, he did not provide the infomation to the NRC in response to questions from the staff about malfunctions in the Auxiliary Feedwater System.

PNO-79-208 South Texas Unit 1 & 2 - Partial Release of Stop-Work Order for Safety Related Concrete Placement - The licensee reported that the stop-work order on concrete placement issued on June 22, 1979, will be partially released to allow placement of all safety-related concrete with the exception of the reactor containment building exterior shell walls.

PNO-79-209 Point Beach Unit 2 - Radiographic Indications in Steam Generator Nozzle-to-Feedwater Line Welds - Review of radiographic results showed defined cracks and indications in two steam generators. The licensee will replace the feedwater-to-nozzle transition pipe on both steam generators.

PNO-79-210 North Anna Unit 2 - Indications in Feedwater Nozzle Welds -

Recent radiographs of the "A" Steam Generator showed three indications in the feedwater. nozzle area. A review of these results is being conducted for code acceptability. Radiographs of the other two steam generators did not identify any prcblems.

PNO-79-211 D. C. Cook Unit 2 - Secondary System Safety Relief Valve Actuation - Public Concern - On July 2,1979, a secondary system pressure perturbation occurred.resulting in the actuation of a secondary system safety relief mive. Although the valve should have closed within two to three minutes, it remained open for about 15 minutes. The initial noise when the valve opened, and the continuing roar of the escaping steam could be heard in Bridgman, Michigan. The plant superintendent informed the citizens via radio and television of the event and assured them that there was no health and safety problem.

j. PNO-79-212 Callaway Unit 1 - News Media Interest in Previcusly Reported Pressurizer Anchor Bolt Support Problem - A news article on the subject matter appeared in the Kansas City Star newspaper. The item was also reported on ABC television news. This metter was initially reported to NRC on March 23, 1979. The licensee submitted a description of proposed corrective action on May 1, 1979.

PNO-79-213 Hope Creek Units 1 & 2 - Fatal Accident on Site - The licensee reported that an employee at the batch plant was found crushed to death in the ccmpactor part of the ice crusher.

PNO-79-214 Oconee Unit 1 - Reactor Shutdown and Related Unplanned Radioactive Release - On July 3,1979, radioactive materials leaked from the Low Pressure Injection heat exchanger. The licensee estimated that 456 microcuries were released, principally cobalt 58.

PN0-79-215 Duane Arnold - IE Bulletin No. 79-02, Revision 1, " Pipe Support Base Plate Designs Using Concrete Expansion Anchor Bolts" -

Preliminary Test Results - The licensee reported the results and conclusions of the testing program required by the subject Bulletin. The results of testing 21 anchors indicated that 40% of the anchors would not meet design seismic requirements. The licensee has suspended testing and has initiated a program for replacement of all anchor bolts.

PNO-79-216 Oconee Unit 2 - Personnel Error - Emergency Condenser Circulating Water System Inoperable - On June 30, 1979, the licensee identified a condition which apparently had existed since May 18, 1979, wherein the Emergency Condenser Circulating Water System had been isolated. The cause of the event appears to be personnel error. Following maintenance in May, 1979, the operator did not properly position five of the six discharge valves. The li.censee has initiated a formal investigation.

PNS-79-69 Quad Cities Units 1 & 2 - Accidental Discharge of a Weapon - On July 2,1979, while guards were cleaning weapons in the gate house, a guard sergeant accidentally discharged a.38 caliber revolver. The bullet struck a bullet-resisting door, but did not penetrate. No one was injured. (Closed) v. PNS-79-70 Surry Units 1 & 2 - Bomb Threat - A bomb threat was received from a person using an on-site telephone. The licensee evacuated and searched certain levels in the Unit 2 containment with negative results. (Closed) w. PNS-79-71 Nuclear Fuel Services, Erwin, TN - Violence During Operators Strike - On the night of July 3, 1979, the homes of several salaried NFS employees and that of the local sheriff were pelleted with missiles in-cluding ball bearings and lead shot. ITEMS OF INTEREST OFFICE OF INTERNATIONAL PROGRAMS WEEK ENDING JULY 6,1979 INTERNATIONAL COOPERATION Action on NRC-USSR Cooneration Procosal IP was advised on July 5 that the Executive Branch had cleared in its final form the proposed NR:.-Soviet Ministry of Power and Electrification " Protocol for the E. change of Nuclear Safety and Regulatory Information and Cooperation in Nuclear Safety Research in Light Water Reactors," and was air-pouching it to the U.S. Embassy in Moscow for hand-delivery. IP has suggested that the Soviets include it as an agenda item for the next session of the US-USSR Joint Committee Meeting to be held later this year. Jaoanese Visit to TMI On July 2, six Japanese Parliamentarians, accompanied by Mr. Kurihara from the Japanese Embassy, visited the TMI observation center and were briefed by Dr. Louis McCabe. Danish Visit On July 5 Mr. Axel Olsen, Deputy Director of the Riso National Laboratory, Denmark, met with the Office of Nuclear Reactor Regulation to discuss safety requirements for research reactors and updated information on TMI. EXPORT / IMPORT AND INTERNATIONAL SAFEGUARDS Astra Reector On July 5 the staff received Executive Branch comments recommending approval of exrort license application XSNM-1428 involving 3.81 kilograms of high-enriched uranium for the Astra reactor in Seibersdorf, Austria. This fur:1 is urgently required in Austria and the staff will forward its review and analysis to the Commission in the near future. NRC-Jaoanese Discussions on Fuel Enrichment Reduction On July 2, Dr. Keija Kanda of Kyoto University in Tokyo and other Japanese officials met with Export / Import and International Safeguards staff to discuss the status of the joint study being conducted by Kyoto University and Argonne National Laboratory leading to the possible reduction in enrichment of research reactor fuel s. Mr. Tem Elsasser, State Liaison Officer in NRC Region I will accompany Commissioner Gilinsky to the National Governors.' Association meeting in Louisville.on July 8. Program Develocment On June 27 and 28 the Region V SLO accompanied Mr. Robert who directs the Washington Radiation Control Program and members of the Program staff on an inspection of the Western and Dawn uranium mining operation in the Spokane, Washington area. The purpose of the visit was to discuss the compatibility of Federal and State uranium mill licensing requirements. Ross Scarano, Chief Uranium Recovery Licensing Branch represents N",SS. State Agreements Nevada Governor List ordered suspension of the license to NECO for operation of the low level rad waste burial. site of Beatty. The action was taken after a truck hauling radioactive waste from Consumers Power, Cobert, Michigan arrived at the site and the driver reported he believed the shipment was leaking. Although surveys by State, DOE and NECO teams at two Nevada truck stops where the driver stopped did not show any contamination, when the shipment was off-loaded, under DOT and State observation, two containers were found to be corroded and leaking radioactive liquid. Dr. Ralph DiSibio, Secretary, Nevada Department of Human Resources will meet with NRC and DOT staff July 13, 1979 in Betnesda. The meeting was arranged by OSP as a followup to Governor List's mee.t1ng with Chairman Hendrie on June 4,1979 in which Governor List requested a special inspection and-compliance program be mounted to assure rad waste shippers comply with regulatory requirements. In response IE, Agreement States and DOT conducted special unannounced joint inspections of 10 licensed rad waste collectors in June 1979. The preliminary results of these inspections will be discussed with Dr. DiSibio along with the status of the Beatty site. Notice of non-Routine Event No. 79-15A was issued July 5,1379, updating the radiography incident in California where a 40 curie Iridium 192 source was picked up and handled by several persons. One individual continues to be hospitalized and two others have suffered radiation burns on their fingers. None were employees of the radiography licensee. 2 Emeraermy Preparedness Acceptance criteria for the essential planning elements for State and local amergency plans are under preparation by the Emercancy Preparedness Staff. Final preparation for a meeting of the ten Federal Interagency Regional Advisory Committees for Emergency Planning has been made. The meeting will be held ir. Kansas City in late July. The purpose of the meeting will be to review plan and exercise evaluation procedures to ensure that all regions are applying the same standards to the State and local governments. Nebraska and Virginia have satisfied all essential elements of NUREG-75/111. Final review and exercise activity prior to concurrence should be coupleted by mid-August. Messrs. Ryan, Collins and Gaut briefed Commissioner Gilinsky on NUREG-75/111 and related emergency preparedness with the States. Messrs. Ryan and Collins briefad Commissioners A:learne and Kennedy on the same matters on July 2. Mr. Ryan and Mr. Gaut met with Commissioner Gilinsky on July 5. A 1:2,500,000 scale map with locations of all licersed facilities, operating and under construction, was prepared for and at th e request of the Hart Sub-comr.ittee Staff. Preparations for Ohio and Wisconsin exercises are under way by Bill Axelson, NRC Region III. He will meet witn Ohio representatives in Toledo on July 9. Other participants at the planning session will be the Radiation Management Corporation (a private utility consulting and service firm) cod the Davis Besse nucleur power plant staff. The reports were produced by MPA and the TERA Corp. and subsequently will be issued on a monthly basis. Correspondence has been summarized by WITS category (work item tracking system) and by Congressman. The purpose of the reports is to provide offices with a data base on NRC's responses to Congressional questions. Ideally the information will tw retrieved to assist ir answering future questions in a consistent fashion and in permitting efficiencies by not recomposing answers that have already been sent to the Congress. FY 1980 The Senate Appropriations Subcommittee on Energy and Water Development marked up NRC's FY 1980 budget request.
