APPENDIX B NOTICE OF VIOLATION Power Authority of the State of New York Docket No. 50-286 Based on the results of the NRC inspection conducted from November 26, 1978 to December 23, 1978, it appears that one of your activities was not conducted in full compliance with NRC regulations and the conditions of your NRC Facility License No. DPR-64 as indicated below. This item is an Infraction.
Technical Specification (TS) 3.2.B.6 states, "Two channels of heat tracing shall be operable for the flow path from the boric acid tanks to the Reactor Coolant System.
The licensee's procedure, S0P EL-7, identifies the Boric Acid Heat Tracing Panel No. 33 for indicating heat tracing temperatures.
This procedure requires that a temperature of 145'F be maintaineu.
Contrary to the above, on December 21, 1978, Circuit No. 63 Meni-toring Line No. 204 from the Boric Acid Flow Indicator to the Manual Baration, Isolation Valve No. 293 was found at 78'F. This line is included in the flow path frcm the boric acid tanks to the Reactor Coolant System. The temperature of 78 F indicated that the heat tracing channel was not operable and that a redundant channel had not been placed in service.
