This raf era to t'.e discussieu between you and I and others of our respective staffe on Juct 25, 1979, regarding the quality of concrete construction cetivities at the Marble Hill Units 1 and 2 plant site.

We understand based on these discussions that Public Service of Indiana vill:

1.

With its contracted organizations, continue surf ace and volumetric exa=ination of existing concrete to estsblish it's adequacy and randomly select and test a statistical sanple, reprc~ Tt..tv e of both congested and other concrete volunes to assure vit 95: reliability and 95% confidence levci, that concrete quality meets requirements.

This examination program ahall be expeditiously impicmented and co=pleted.

Evaluate and taka appropriate corrective actions of all identified deficiencies and assess them in terms of their impact en safety reinted concrete construction activities.

2.

Identify and evaluate concrete repair areas for adequacy.

This effort is to include no less than those areas re'e.r.nced by existing QA/QC documentation for repaired areae end those identified by 100% visual /nechanical exa=1 nation of existing concrete structures.

Limit future concrt :e place =ent to non-safety related structures to pcInit the NRC to review and evaluate QA progra 1.=provements instituted on or about May it, 1979 by PSI as a result of previously identified concrete deficiencies. During the existence of this concrete placement limitation. Not resume contrata plac= ment for safet9 related structures until the NRC is satisfied by co=prehensive de=enstration that your upgraded QA/QC program and process controls are adequate.

Following resu=ption of concrete placement for safety re3ated structuras, provide complete (100%) " overview" of all safety related concrete activities at the site.

This " overview" is to continue until adequate conf.1dence is established to the satisfaction of PSI and NRC.

Evidence of PSI's ovarview of the involved contractor's quality re. lated activities is to be documented.

6.

Stop all safety related concrete activities until the cause and consequent conditions are fully rectified if significant deficiencies are identified during the course of cc=pleting the above actions.

NRC is to be 1-eciately informed of such occurrences.

Please inform us if your understanding of this program is different from that stated.
