Box 468 Richland, Washington 99352 Docket No. 50-509 flotice of Violation Based on the results of an NRC inspection conducted during the period of April 17-20, 1979, it appears that one of your activities was not conducted in full compliance with conditions of your flRC Facility License flo. CPPR-155 (Unit 5) as indicated below.
10 CFR 50, Appendix B, Criterion V, as implemented by Section 17.1.5 of the Quality Assurance Program documented in the PSAR and the K PSS QA Pro-gram Manual procedure QAP-2, Paragraph 3.1, states, in part, that, " Activities affecting quality...shall be accomplished in accordance with... instructions, procedures, or drawings."
Paragraph 14.02 of Ebasco Specification No. 3240-415, " Testing and Inspection Requirements for Concrete Materials and Concrete," states, in part, that, "...in-process tests on grout used for structural pur-poses shall be made in accordance with Table 2."
Table 2 requires such grout to be tested by the flow cone method, "... daily during production or every five cubic yards, whichever is more frequent."
Contrary to the above, six cubic yards of grout utilized for structural purposes on April 18, 1979, in Unit 5 placement RBF-002, 004-351 were not tested by the flow cone method.
This is a repeat infraction.
