Dear Mr. Spencer:

This letter transmits additional information regarding our responses (References 2 and 3) to your letter of January 22, 1979, which discussed the results of the inspection conducted between fiovember 13 and December 19, 1978, of activities authorized by fiRC Construction Permits flos. CPPR-154 and CPPR-155.

The additional information supplied by this letter was verbally relayed to your inspector D. F. Kirsch and elaborates upon the reference letters. The following nine items of corrective action have been implemented:

Item 1: Requalify PTL Site Technicians - Recertification of PTL personnel was started on flovember 17, 1978, and was completed on December 11, 1978. An engineering analysis of concrete testing performed prior to fiovember 17, 1978, concluded that no adverse effect on the quality of con-crete placed resulted from any absence of technician certification.

Item 3: Review of Soils Test Records - The soils testing records are not nuclear safety telated.

Item 4: Completion of Test Records _ - Test recards for PTL field tests were reviewed by PTL and Engin'.ering for accuracy and completeness with all undocumented nonconforming test results being resolved.

Item 5: Review / Revision of PTL Certification and Training Program -

Two separate reviews were conducted of the PIL program by PTL and WPPSS/Ebasco with the intent of strengthening the program in light of the extensive findings. Both reviews concluded that if the program remains properly implemented, it will continue to be adequate.

Tio further revision to the PTL program is anticipated at this time.

Item 6: Development of a Preventative Maintenance Program - PTL committed to a preventative maintenance program which entails a check on field equipment at the time of re-calibration. The calibration procedure has been revised to include the preventative maintenance requirements and has been implemented.

Evaluation of Consequences of Tests Performed by Improperly Item 7: ~ Trained Personnel - An evaluation of the consequences of potentially improperly performed tests was performed by Engineering for both concrete tests and soils tests. The results of this e 11uation are documented in a report which conclusively establishes by statistical means that no adverse reaction or degradation of the strength or durability of the concrete is probable.

Item 8: Submittal of All Testing Records Within One Week - The PTL document control system was completely reorganized to preclude reports of tests being delayed by the system. The new system has been in operation since December 18, 1978, and revised for viability in May,1979.

Spencer Page 3 14RC Inspection of WiiP-3 And W!iP-5 Docket tiumbers 50-508 And 50-509 Item 9: PTL Corporate QA to Increase Frequency of Audits _ - PTL has committed to increasing the frequency of their internal audits of PTL site lab operations from once per year to once per quarter. The first audit urder this program was scheduled for February,1979, and was conducted on January 29 and 30, 1979.

In addition to the PTL Corporate QA audits, WPPSS/Ebasco QA a'id the Cement and Concrete Reference Laboratory (CCRL) have audited and reviewed, respectively, both PTL (site) and PTL (Seattle) operations.

In addition to the items above, the PTL Site Manager was removed from his position on Ilovember 17, 1978, and replaced by the Assistant QA Manager from the PTl Pittsburgh Office until a new Site fianager was brought in to replace him on December 11, 1978. PTL site operations administration responsibility has been removed from the Seattle District Manager and reassigned to report to the Pittsburgh QA Manager.

These actions have all been implemented and we consider the above responses satisfy the commitments in the referenced letters.
