50-498/79-08 50-499/79-08 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted during the period May 15-18, 1979, it appears that certain of your activities were not conducted in full com-pliance with the conditions of your NRC Construction Permit No. CPPR-129 as indicated below:
Failure to Have a Procedure for Monitoring Welding Operations Criterion V of 10 CFR 50, Appendix B, requires that activities affecting quality shall be prescribed by documented instructions, procedures or drawings, of a type appropriate to the circumstances.
The Pittsburgh Des Moines (PDM) Steel Company Quality Assurance Manual, paragraph 5.11 states that tha site QA shall sufficiently monitor the welding operation to assure conformance to procedures and specifications.
Section 17, paragraph 17.15A of the PSAR establishes the requirement that activities affecting quality shall be prescribed by doci:mented instructions or procedures by contractors performing such activities.
Contrary to the above:
On May 16,1979, the IE inspector determined that monitoring of Reactor Coolant System Support welding operations was being accomplished using a monitoring program that was not documented in any instruction or procedure. This was confinned in discussion t'th PDM QA personnel.
This is an infraction.
