STN 5]-482/79-04 Appendix A NOTICE OF VIOLATION Based on the results of the NPC inspection conducted daring the period March 5-8,1979, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construction Permit No. CPPR-147 as indicated below:
10 CFR Part 50, Appendix B, Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."
Failure to Initiate Nonconformance or Deficiency Reports Daniel Procedure QCP-I-04, Rev. O, paragraph 3.5 states in pact,
" Discrepant conditions which cannot be corrected during in-process inspection shall be documented and controlled using a Noncon-formance or Deficiency Report...."
Contrary to the above:
Temperatures in the concrete lab curing room fell below the ASTM C-31 minimum tolerance of 70.4 F for three periods of time and the recorder failed to record temperature data for one period of time, as listed below, and Nonconformance or Deficiency reports were not initiated.
