'50-445/79-06 50-446/79-06 Appendix A Notice of Violation Based on results of the NRC inspection conducted during the period March 1-30, 1979, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Construc-tion Permit No. CPPR 126 as indicated below:
Failure to Follow Inspection Procedure for Inspection of Class IE Cable Tray Supports 10 CFR 50, Appendix B, Criterion V requires that activities affecting quality shall be accomplished in accordance with prescribed procedures, instructions, or drawings.
Brown and Root Procedure QI-QP-11.3-2, Revision 7 requires that field welding inspection and installation / erection inspection, including location and elevation, be accomplished on all Class IE cable tray supports.
Gibbs and Hill Drawing El-0602-03-S, Revision 2 indicates that cable tray supports 2705 and 2773 are Class IE supports.
Contrary to the above:
On March 28, 1979, review of Operational Travelers ELE 79-CTH-2705 and ELE 79-CTH-2773 indicated that the field welding and location /
elevation portion of the inspection were not accomplished for these two Class IE cable tray supports.
These portions were signed off as "not applicable." The cognizant Brown and Root electrical inspector advised the IE inspector that these supports had been inadvertantly inspected as non-Class IE.
This is an infraction.
