EVENT DATE 6-21-79 REPORT-DATE 7-20-79 ATTACHMENT CAUSE DESCRIPTION AND CORRECTIVE ACTIONS:

The engine oil pressure switch (0-PS-4795) was erroneously left isolated following the conduct of a preventive maintenance calibration on 6/20/79.

T:lis isolation also isolated other lube oil pressure switches that disabled the diesel starting circuit. The procedur was deficient in that there was no step to ensure the instrument was returned to :ervice with the isolation valves in the proper lineup. PM's that put equipment out of service are followed by an operational test before the equipment is declared back in service. The PM on the engine oil pressure switch was not recognized as putting the other pressure switches, and thus the diesel out of service, so no operational test of the diesel was conducted following the PM.

The PM procedure has been changed to include ~ the engine oil pressure switch as a PM that places the diesei out of service. In addition, all instrument, electrical and mechanical PM's, functional test procedures and maintenance procedures have been reviewed for, and changes made to include steps, where needed, to return equipments to service following the maintenance action.

T raining sessions have been conducted to ensure technicians recognize the f.eed for verifying that equipments are returned to service in the proper line-up following a maintenance action.
