Gentlemen:
On June 14, 1979, SCESG identified to Region II the apparen t failure :o implement Quality Assurance requirements on N mco limit switches by the manufacturer, Acme-Cleveland Corporation of Jet f erson, Ohio.
On June 27, 1979, a copy of a letter received by SCE&G f ror Uestinghouse detailing a possible difference bettaen actual and apparent steam generator

T he purpose of this letter is to provide t he SCESG report rela t iv e to these matters and to update the status of previously identified i r es;.
Namco Limit Switches - On June 16, 1978, NESG identifiec to the NRC 3 failure of a valve supplier's "sub ontractor procurement con t rol sys tem",
- tich resulted in contracts being placed with suppliers of valve attachne 't hardware without appropriate QA controls.
As part of the corrective actian to re30lve the imprope procurement control, SCESG accompanied the valve supplier, as an observ<r, to sescral suppliers including Namco Controls. This activity confirmed tha Namco did not _ have a Quality Assurance program meeting all the appropriate criteria of 10CFR50, Appendix B, and that no customer of Namc o had ever imposed such a requirement by purchase order.
Inmediate effort was initiated by FCESG to resolve the fact that the Nanco QA program fc i l ed t o provide objective evidence of manuf acturing repeatability of the limit suitches.
This concern is relevant to the IEEE qualitication even though there appt ars to be a large margin of inherant design con,;ervatism in the basic swi tch,

Mr. James P. O'Reilly Page 2 July 10, 1979 Resolution of the tbove concerns were pursued on all SCE&G Namco switches by two parallel actions:
1) de :lopment and inplementation of inter ium QA program measures which would assure repeatable new switches; and 2) inspection by Namco of a statistically significant sample of SCE6C switches already shipped to assess inheran: (but not verifiable) existing QA Program control and determine significance to the hardware, were the condition to have re-mained uncorrected. This information was pre ided to Region 11 in our December 22, 1978 report.
The interium Namco QA Program was developed and approved by February 7, 1979. All suppliers of SCE&O hardware using Namco switches ordered replace-ment switches contractually imposing the interim program.
On June 12, 19 79,,SCESC received in forma tion from one of the valve suppliers that Name may have f4. led to comply with all of the newly imposed contractual requirements and shipped non-conforming replacement switches. This information was confirmed on June 14, 1979 by the Pres id en t of Namco and three customers of Namco were indicated as haviag received non-conforming switches.
As immt diate corrective action, Nam o requested the return of all switches from the three customers.
In addition, a 10CFR21 report dated June 14, was provided to NRC Headquarters by "am.co.

Our update of previous reported items is as follows:
1 Pipe Suoport Loadings, 'Je lan _yal v e s - Re-evaluatica o f the piping system will be completed in two to ti.ree weeks and new support designs, where required, will be completed in about a month. Completion of the reluired field changes will depend on the availah' Lity o f supports.
When the re-design is completed, a final report will be issued.

Electrical Suppc>t Splicg - The reinspection prograrr established to detect def ective splices is approximately 30% complete. To date, the samples inspected have fallen within the acceptance limits of the established sampling plans. Three defective splices have been detected during this program which are documented for repair. The reinspection effort is es-timated to be completed in late August; barring samples that fall outside the acceptance criteria whi-h would predicate fur.'her inspec* ton effort.
A final report will be issued at the completion of this ef fort.
