SUMMARY

OF KEY EVENTS Week Ending Ngv 25, 1979 Indian Point Units 2 and 3 The NRC staff met with Consolidated Edison Company of New York and the Power Authority of the State of New York on May 22 and 23,1979. The seismic reanalyses for safety-related piping in the Indian Point Nuclear Generating Plant, Units 2 and 3 was discussed. The staff concluded that the plant could continue to operate pending submittal of additional information by May 31, 1979.

Northwestern University Northwestern University has withdrawn their application for construction pemit and operating license for an AGN-201 nuclear research reactor from O' mn State University.

Palisades On May 20,1979, Palisades was returned to operation after performing pipe support modifications to resolve the potential seismic desi'gn probl ems.

Meetino on Regulation of Hazardous Wastes NRC met with EPA staff to coordinate the development of regulations for management of both EPA regulated hazardous wastes and URC regulated icw-level radioactive wastes.

It wa's agreed that the NRC staff would identify specific areas of concern that might be amenable to solution by a t:00.

A preliminary draft for joint agency staff review will be available in about three weeks.

White Mesa FES Issued NMSS issued a Final Environmental Statement related to operation of the White Mesa Uranium Mill proposed by Energy Fuels Nuclear, Inc.

The FES concludes that the proposed mill should be-licensed subject to certain conditions for protection of the environment.

Philicoine Reactor Excort Westinghouse indicates that Philippine officials are discussing possible cancellation of the Philippine Nuclear Power Plant project due to lack of ability to obtain equipment and mater %1 frcm the U.S.

Emergency Preparedness Final crafts of the Nebraska and the Virginia Radiological Emergency Response Plans have been reviewed and will be reviewed by the Regional Advisory Committees by the end of June, bb2 b

DIVISION OF RULES AND RECORDS Week Ending May 25, 1979 The Executive Director for Operations has issued an amendmerit to the NRC Notices of Systems of Records to add a new system, Oral History Program, NRC-25. The purpose of the system is to develop a collection of taped interviews with NRC staff members, former agency personnel, and other in-dividuals who have been involved in nuclear regulatory activities. These interviews will be transcribed for use as a resource for the future writing of a history of nuclear regulation.

They will provide an important tool for documenting the role of the NRC and a valuable resource for present and future researchers. The amendment to the NRC Notices of Systems of Records becomes effective 30 days after publication in the Federal Register.

The Executive Director for Operations has issued amendments of NRC system of records Radiation Exposure Information and Reports, NRC-27, to include records of individuals who may have been exposed to radiation or radioactive materials off-site from a facility, plant, installation, or other place of use of licensed materials, or in unrestricted areas, as a result of an incident involving byproduct, source, or special nuclear material. The amendments also clarify the paragraph entitled " Routine uses of records..." This clarification does not add any new use or intended use of the information in the system, but only states that records of individuals who may have been exposed to radiation are part of the data that may be provided to others under the system's routine use provisions. The amendment. nere published in the Federal Register on May 21, 1979 and became effective on May 21, 1979.

The FY 1979 Selective Reinvestigation Program is presently underway.

A nemorandum frem the Division of Security to t administrative offices of the various NRC Offices will identify individuals who are to conplete the necessary security reinvestigation forms.

NRC Manual Chapter 2101, Part IV " Control of Official Use Only Infor=ation," was published and disseminated to NRC offices, divisions and branches.

The proposed NRC Regulatory Guide " Protection of Classified Matter and Information," setting forth methods to be used by licensees in t=plenenting the provisions of 10CFR Part 25, " Access Authorization for Licensee Personnel" and 10CFR Part 95, " Security Facility Approval and Safeguarding of National Security Information and Restricted Data,"

has been prepared for forwarding to Directors of Offices and Divisions and Regional Offices for concurrence or cecments.

NRC Security, DOE International Affairs and Executive Legal Office personnel participated in joint N?.C/ DOE-UK discussions regarding the protection of UK sensitive infor=ation including classified, given to the United States.

The Division of Security coordinated protective services for the Atomic Safety and Licensing Board Panel at Plymouth, Massachusetts on May 23, 1979 when due to the possibility of demonstrations against nuclear power occurring in conjunction with the hearing.

On May 22, 1979 a pre-survey meeting was held with representatives of the National Telecc=und cations and Infornatica Administration ( NTIA) to brief them on the NRC c rganization, functions and responsibilities. NRC clearances have been granted to NTIA representatives and badges issued for convenience of access to NRC tacilities during the survey geriod.

OFFICE OF NUCLEAR REACTOR REGULATION WEEKtY ITEMS OF INTERE3T (Week Ending May 25, 19791 Fort Calhoun On May 18,1979 the Omaha Public Power District (0 PPD) reported that CE tests of a Fort Calhoun surveillance capsule indicated the neutron flux at the reactor vessel wall was 2.5 times the level used in SAR calculations. OPPD has, therefore, initiated more conservative heat-up and cocidown procedures end will propose revised Technical Specifications We are reviewing by June 8,1979 to accommodate the new information.

Indian Point Units 2 and 3 The NRC staff met with Consolidated Edison Ccmpany of New York and the Power Authority of the State of New York in Bethesda, Maryland on May 22 and 23,1979.

The purpose of the meeting was to consider additional information supplied by these licensees with regard to the seismic reanalyses for safety-related piping in the Indian Point Nuclear Generating Plant, Units 2 and 3.

Unit 2 has a total of eight lines that used algebraic sumnation.

Three of these had been reanalyzed by the SRSS method and remained well within allcwable code.

Five of the lines remain to be reanalyzed.

One of these lines, line 70 (Pressurizer Relief Line) will be reanalyzed on an expedited basis by Friday, May 25, 1979.

Since the FSAR design basis was to combine loads by the absolute basis, the licensee will, at our request, add a compensating 30% to the seismic portion of the loading in performing the reanalyses.

(The 3 lines already analyzed will stili be within allcwable stresses within this 30% factor anplied).

Unit 2 is scheduled for refueling in June 1979. All remaining 5 lines will have been reanalyzed before restart.

The staff concluded that continued operation is warranted on the above basis until the June refueling outage.

Unit 3 has a total of 269 safety-related line/s that used algebraic summation.

Fifteen of these lines have been reanalyzed using an SRSS method and remained within allowable limits using that method.

Based on additional information discussed at the meeting, the staff concluded that the plant could continue to operate pending submittal of additional information by May 31, 1979.

That information includes an agreed upon method of reanalysis that appropriately reflects the results that would be achieved by absolute scanation of loads.

PASNY must also detail to the NRC its degision criteria for threshhold acticn if calculated stresses exceed allcwable or actual piping configuration differs f rcm the drawing configuration.

. Northwestern University Northwestern University has withdrawn their application for coastruction permit and operating license, by letter dated May 15, 1979.

Northwestern University was to obtain an AGN-201 nuclear research reactor from Oregon State University.

Salem Unit 1 On May 14,1979, the ASLB for the proposed expansion of the Salem Unit 1 SFP storage capacity issued a notice that the hearing which recessed on May 4, 1979, will reconvene on July 10, 1979 at Salem, New Jersey.

Contentions remaining involve possible alternatives to the proposed modification.

Also to be discussed are board ouestions regarding the potential impact on an expanded Salem SFP due to an accident similar to that which occurred at Three Mile Island 2.

Palisades On May 20,1979, Palisades was returned to operation after performing pipe support modifications to resolve the potential seismic design problems reported in the May 4,1979 weekly Information Report.

The seismic design concern was not related to the problems in IE Bulletin 79-07.

I Surry and Beaver Valley Following extensive discussicn's between the staff, the licensees and Jtone and Webster, meetings were held on May 24,1979 among these parties at which the staff defined acceptable factors of conservation to be ir.cluded in these portions of the reanalysis which will utilize soil-structure interac' tion.

The licensees have not yet determined the schedule impact's caused by the staff decisions in this matter but are expected to do so shortly./

Maine Yankee The Commission. issued a Termination of Order to Shew Cause for the Maine Yankee. plant on May 24, 1979.

This Order allows the facility to return to power operation.

Millstone 1 On January 25, 1979, while performing routine control rod surveillance, control rod 22-07 stuck between position 46 and 48.

Efforts to free the control rod during plant operation was unsuccessful.

Analysis was performed in accordance with the Technical Specifications to shcw that safety margins with the control rod stuck in the nearly fully withdrawn position were acceptable.

Following reactor shutdown for refueling on April 28, 1979 it was determined, after separate removal of the ' control rod and the drive mechanism, that the problem was in the control rod drive and not in the control rod or velocity limits. Disassembly of the control rod drive revealed that a piece of a neutron source tube that had broken off during or prior to removal in 1976 had lodged in the drive notch.

The drive had to be forced apart to free the source piece which is reportd to be about 1" by 1/2" in measurement.

THe licensee will submit a supplement to the original Licensee Event Report (LER) submitted following the accide'nt.

The supplemental report will provide the details of the incident that caused control rod 22-07 to stick and discuss generic aspects.

OFFICE OF STANDARDS DEVELOPMENT IMPORTANT EVENTS FOR THE WEEK ENDING MAY 25, 1979 1.

Meeting with Staff from EPA Offices of Solid Waste and Radiation Programs: On Thursday, May 17, 1979, D. Smith, T. Johnson, and C. Bishop of LLWB, R. Fonner of ELD, and E. Conti and R. Grill of SD met with EPA staff members from the Offices of Solid Waste and Radiation Programs to coordinate the development of regulations for management of both EPA regulated hazardous wastes and NRC regulated low-level radioactive wastes. Areas of dual responsibility, i.e., when the two waste forms are mixed, or when one applicant may require permits or licenses from both agencies, were identified. The comments supplied earlier by NRC on the EPA proposed Hazardous Waste Regulations were also discussed.

It was agreed that the NRC LLWB staff would identify specific areas of concern that might be amenable to solution by a MOV. A preliminary draft for joint agency staff review will be available in about three weeks.

TMI Area Pooulation Enumeration Census: Data collection among residents living within five miles of TMI will begin in the next few weeks. HEW is funding this project in cooperative agreement with the State of Pennsylvania, the purpose of which is to enumerate the 62,000 residents in the area. At the request of, CDC, Robert Goldsmith of the Radiological Health Standards Branch has assisted in developing the survey quesitionnaire for the door-to-door interviews.

The survey will concentrate en obtaining information necessary for possible follow-up studies, and will include questions pertaining to pregnancy, cancer prevalence, cigarette smoking and evacuation from the area.

In addition to the population enumeration, several other studies of the residents around TMI are planned by the State of Pennsylvania and HEW.

4 0FFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending May 25, 1979 White Mesa FES Issued The Uranium Recovery Licensing staff has issued a Final Environmental Statement related to operation of the White Mesa Uranium Mill proposed

.by Energy Fuels Nuclear, Inc., for location near Blanding in San Juan County, Utah.

The 2000 TPD mill will dispose of tailings in impoundment cells built, filled, and reclaimed in stages.

The FES concludes that the proposed mill should be licensed subject to certain corditions for protection of the environment.-

_ Low-Level Waste Branch Position Incorporated Into Licenses NRC and South Carolina have. incorporated the Low-Level h'aste Branch position:

on site stabilization and cicsure into the NRC and State licenses for the Barnwell site.

The objectives of the branch position have been provided to the ASLBP which is addressing clo.sure of the Sheffield sitec St.aff is meeting with Illinois state officials and is starting

. work to develop a technical site closure plan for Sheffield.

Staff is also meeting with Washington State officials to incorporate the branch position i'nto the NRC and State Richland licenses.

We plan to work with Nevada and State Programs to add similar requirements to the Beatty license.

Presentation by Hench, University of Florida On May 15,1979, Waste Management Division staff attendad a seminar,given by Larry Hench of the University of Florida in whi h he reviewed testimony 9

he was to give to Congress the following day.

Hench has performed NRC-sponsored experiments ~. comparing the leachability of various ceramic materials and presented these results as well as his personal ranking of ceramic materials for waste containment.

Physical Security Profile Ouestionnaire As part of the ongoing study of the potential threat to nuclear activities from insiders by the Physical Security Development Branch, this office has developed a physical security profile questionnaire for use in the study.

The questionnaire will assist in gat'hering data c'n security systems used by industries analogous to the NRC with regards to the insider threat.

Itecs of Interest 2

Transient Shipments A draft Commission Paper on Transient Shipments has been circulated for staff comment.

It is being revised based on comments received from other NRC offices.

The revised draft paper will be completed before June 1 and will be circulated for office concurrence.

Spent Fuel Rule Publication Preparations are being made for the publication of the Federal Reoister Notice concerning the interim rule regarding safeguards of spent fuel shipments. Also the draft guidance document to support licensee imple-mentation of that interim rule is being prepared for publication.

Nuclear Fuel Services-Erwin Upgraded measures for the control of both personnel and high enriched uranium have been implemented recently.

These measures include strengthening controls to thwart falsification of records and un-authorized use of seals by a single insider. Additionally, storage and process control measures have been incorporated into the safeguards program, effective June 1,1979, to provide weekly information for the most attractive form of SNM in two of three areas within the HEU plant.

The third area involving equipment modification is targeted to have process monitoring controls in place by August 1,- 1979.

US/IAEA Acreement When approved, the US/IAEA agreement will require extensive data reporting to the IAEA by both DOE and NRC. A joint DOE /NRC meeting was held on May 23, 1979, to discuss the interface and the cooperation necessary between both agencies in utilizing the NMMSS information systempto provide the data required by IAEA upon implementation of the US/IAEA agreement.

In a related activity, NMSS staff attended a working meeting at Oak Ridge, Tennessee, with the Oak Ridge Operations Office of DOE and Union Carbide, the operators of the NMMSS system.

The purpose of the meeting was preparation of licensee reporting instructions and the design and construction of the necessary software to permit use of NMMSS in providing the data required by the US/IAEA agreement.

OFFICE OF INSPECTION AND ENFORCEMENT Items of Interest Week Ending May 25, 1979 1.

Preliminary Notifications relating to the following actions were dis-patched during the past week:

a.

PNO-79-67A0 and 67AP Three Mile Island Unit 2 - Nuclear Incident at Three Mile Island Unit 2 - PNO-79-67A0 was issued to provide updated status infor nation regarding the incident.

PNO-79-67AP provided information relating to the reported death of a number of cows in a herd located near TMI. The NRC, EPA, FDA and the State of Pennsylvania are investigating.

Initial findings are that the deaths are not related to the TMI accident.

b.

PNO-79-ll5 Surry Units 1 & 2 - Potential Overstress Caused by Actual Weight of Vela 7 Valves - Westinghouse informed the licensee that the actual weight 3f six safety-related valves was greater than that used in the piping itress analysis. The valves are 12-inch motor operated valves located in the discharge line of the safety injection accumu-lators. The effect of this errcr on the piping stress ons. lysis is not known at this time, but is being investigated by the licensee and Westinghouse.

PNO-79-ll6 Surry Unit 2 - Worker Injury - A Daniels Construction Co.

worker was injured on May 17 when he fell into the upper portion of a steam generator. The injury was not serious but the individual was sent to the hospital for examination.

PNO-79-ll7 Rancho Seco - Startup Delay Because of Main Electrical Generator Damage - While performing a final inspection on the main electrical generator, a Westinghouse representative detected two burned spots on the generator stator coil. The licensee decided to disassemble and repair the generator. Repair time was estimated to be from 17 to 24 days.

PNO-79-ll8 Davis-Besse Unit 1 - Inoperable Hydraulic Snubbers - The licensee reported that of 97 Grinnell hydraulic snubbers tested, 68 failed to meet the acceptance criteria for bleed rate and/or lockup.

The major problem was a low bleed rate which Grinnell attributed primarily to air contamination in the snubbers. The air contamination was tenta-tively traced to fabrication and original installation techniques. The licensee plans to test and repair, as required, all safety-related snubbers and is also reevaluating the acceptance :riteria. Region III (Chicago) will folicw this, matter to resolution.

PN0-79-120 D. C. Cook Unit 2 - Plant Shutdown Because of Pipe Cracks in Feedwater System - The reactor was shut down on May 19 to investigate nonradioactive leakage of about three gallons per minute in the contain-ment building. The licensee's inspection revealed circumferential cracks in Nos. 1 and 4 main feedwater lines approximately 5/8 inch upstream of the weld connecting the feedwater lines to the steam generator nozzles.

The licensee indicated that the reactor will be out of service for at least one week. An inspector knowledgeable in piping and NDE was dis-patched to the site to review the matter.

PNO-7E-121 Surry Unit 2-Onsite Contaminated Water Spill - On May 21, contaminated water spilled to the floor of the Unit 2 safeguards building, out a door and onto a concrete pad and the ground.

Decontamination of the exterior concrete pad was completed on May 21. No personnel contamination or overexposure resulted from this event.

i. PN0-79-122 Sequoyah Units 1 & 2 - Stress Analysis of Piping Supports -

The licensee reported on May 22 that a TVA review of safety-related piping support design indicated that summation was not made of the static, thermal and safe shutdown earthquake forces at the Sequoyah facilities.

Unit 1 is complete and awaiting issuance of an operating license; Unit 2 is under construction and is about 80% complete. The largest force was apparently used in the design of piping supports instead of an appropriate sumation of all forces.

Initial indications are that safety-related systems are involved. The effect of this problem on schedules was not known. An IE inspector was dispatched to review the problem.

j. PNO-79-123 D. C. Cook Unit 2 - Feedwater System Pipe Cracks - This Pre-liminary Notification supplements PNO-79-120.

Following detection of cracks in the feedwater piping of Unit 2, the licensee expanded his nondestructive examination to include the Unit 1 feedwater system.

Similar cracks were found in Unit 1.

The cracks appear to be originating at the pipe interior surface and propagating to the outer surface. The licensee has removed samples from the pipes for analysis. The licensee plans to perform additional ultrasonic testing and is fonnulating a NDE program for f

other feedwater line welds.

PNO-79-124 Zion Unit 1 - Inadvertent MSIV Closure and Safety Injection -

On May 23, 1979, while operating at full power, Unit i experienced a Main Steam Isolation Valve (MSIV) isolation, a reactor scram, and a safety injection. An imediate check of major plant and operating parameters verified that the above events were not due to actual plant conditions.

The NRC Resident Inspector was present in the control room and also veri-fied that the event was due to spurious signals.

It was determined that the event was caused by human error when an operator inadvertently skipped one page of a checklist in the performance of a surveillance test of the safeguards logic of train "A".

IE inspectors at the TVA design office recorted that the incorrect analy-sis was limited to the Sequoyah Units and had not been utilized at other TVA facilities.

The Commission is currently reviewing several proposed exports for this project.

President's Executive Order on Foreton Environmental Imoacts A small interagency group was convened on short notice by State on May 22 for the purpose of reviewing a second version of the proposed draft unified procedures prepared by State in accordance with Executive Order 12114 concerning foreign environmental effects of nuclear export activities.

Joe Lafleur and Hans Schechter attended from NRC.

The updated draft procedures are expected to be distributed by State within the next few days, at which time copies will be forwarded to the Commission and other NRC offices for comment.

Meetina with IAEA Inscection Personnel On May 21 an informal meeting was held at State to permit State, ACDA, DOE, and NRC (IP and NMSS) representatives to discuss with a member of the IAEA staff U.S. activities to assist other countries in developing effective national systems of material control and accounting.

The activities of the U.S. Government, especially as mandated by section 202 of the NNPA, to provide assistance to other countries, were discussed. The activities of the IAEA to provide this type of safeguards assistance, along with the factors which they must consider,were aise discussed.

It was agreed that further efforts to coordinate these activities would be useful.

Meetino of the U.S. Interacency Action Plan Working Group on International Safecuards NRC representatives (Ken Cohen IP, and Ben Easterling, NMSS) participated s

in a regularly scheduled meeting of the APWG on May 22 at the State Depart-ment. The principal foc": v' the meeting centered on a review of the Group's Work Plan Iniplementation Status Report.

2 INTERNATIONAL COOPERATION Meetings on Protecting Information Received from the U.K. in Confidence On May 21 NRC representatives (Ken Cohen, IP; Ray Brady, Bob Whipp, and Bob Brady, Security) participated in an interagency meeting at the Old Executive Office Building with a delegation from the U.K. interested in discussing (1) protection of U.K. information held in U.S. custody; (2) the impact of the FOIA and E.0.12065 on the protection of such inforination; (3) the role of the Information Security Oversight Office; (4) the release of U.S. historical documents held in U.S. custody; and (5) procedures for pro-tection of unclassified U.K. information provided in confidence. The U.K.

delegation was scheduled to be in the U.S. through May 29 meeting the various U.S. agencies separately on these and other subjects of interest. The NRC met with the U.K. delegation on May 24 at DOE; Howard Faulkner attended from IP.

Commissioner Gilinsky, IE, SP, NRR, GCM and OPE Staff members met with a four county group, led by Mr. A. Del Bello, County Executive of Westchester County, New York. Other counties represented were Orange, Putnam, and Rockland. The four county group had previously requcsted a meeting with Chairman Hendrie who

was unable to attend the meeting on May 23, 1979. Basically, the group asked for

information relating to the safety of the Indian Point Nuclear Station and funds to develop a model emergency response plan and system for the four counties.

It was agreed that NRC would provide the four county group with information they required concerning the Indian Point facilities. The four county group was introduced to Mr. Theodore A. Rebelowski, Resident Inspector at Indian Point, as an NRC point of contact in their area. Mr. Del Bello indicated that Mr. David Smith of this office would serve as a point of contact for the four county group. Commissioner Gilinsky indi'cated he could make no commitment on any funding at this time but if the grcup wanted to put down in writing their specific ideas as to what they had in mind, the NRC would take a look at it.

Other discussion ensued concerning emergency preparedness in the Indian Fe'at Area, and Federal, State and local government relationships and on going programs.

It was pointed out by Collins and Elsasser of SP, that several people in the four counties, had received NRC training related to emergency planning.

FY 1979 Reprogramming The reprogramming request sent to Congress on April 26 has been deferred by the House Appropriations Committee, Subccmmittee on Energy and Water Development pending complete review of the FY 1979 funding implications of TMI.

The review of the FY 1979 funding implication of TMI is scheduled to be a part of the mid-year financial review.

The BRG and EDO are scheduled to be briefed on this financial review next week with a subsequent SECY paper to the Commission for approval.
