2 S MAR 1978 APPENDIX A NOTICE OF VIOLATION Metropolitan Edison Company Docket No. 50-320 License No. DPR-73 Based on the results of the NRC inspection conducted on February 22-24, 1978, it appears that some of your activities were not conducted in full compliance with conditions of your NRC Facility License No. DPR-73 as indicated below. Both Items are Infractions.

A.

10 CFR 50, Appendix B, Criterion V states in part:

" Activities affecting quality shall be prescribed by... procedures... and shall be accomplished in accordance with these... procedures...."

The accepted Operational Quality Assurance Program (FSAR Section 17.2.7) states in part:

...The procedures which are the Operating Quality Assurance Program's detailed requirements, are... as shown in Table 17.2.2..."

Table 17.2.2 refers to Administrative Proced-ures.

Administrative Procedure (AP) 1022, Control of Test and Measure-ment Equipment, Revision 7. January 17, 1978, Section 3.10.6 states in part:

"If a piece of test equipment... has become uncalibrated a hold sticker shall be placed on the instrument and it shall be placed in a designated storage space such as Hold for Repair / Calibration Area..."

Contrary to the above, on February 23, 1978, Torque Wrenches MEC-10, MEC-7 and MEC-32 were out of calibration (uncalibrated);

were stored in Locker B (designated as a. storage area for calibrated instruments); and, did not have a hold sticker attached. The calibration due dates as indicated by the stickers attached were July 5,1976, December 1977 and December 1977, respectively.

2. 1, Section 5.1 of Administrative Procedure AP 1001, Document Control, Revision 12, November 8,1977 states in part:

"All... drawing revisions... will initially be received by TMI Administrative Office. A designated individual...

will assure that said documents are properly distributed in accordance with... distribution list. All drawings...

distributed in accord:nce with this list will be considered controlled drawings. These drawings will be stamped controlled cooy in red..."

2 9 MAR 1978 Contrary to the above, controlled drawing revisions were re-ceived (as indicated by the Master Drawing Index) and not dis-tributed in accordance with the distribution list (TMI Nuclear Station Unit 2 Drawing Distribution, January 1, 1978) in that on February 23, 1978:

Two out of seven controlled drawings selected in the In-a.

strument and Control (I&C) shop - 2517 Revision 8 and 3020 sheet 4A Revision 6 - did not correspond to the latest revision indicated by the Master Drawing Index.

One out of the seven drawings, 3044 sheet 1, could not be located within the I&C shop, b.

Two out of four controlled drawings selected in the Mechanical Maintenance Trailer (MM) - 2414, Revision 15, and 2076 EF sheet 1, Revision 7 - did not correspond to the latest revision indicated by the Master Drawing Index. One out of the four drawings, 2517 Revision 8, was not on the distribution list for MM, was not the latest revision (i.e. Revision 9), and was stamped controlled cooy in red.

One out of six drawings. 3094 sheet 134 Revision 6, select-c.

ed in the Electrical Maintenance Trailer did not correspond to the latest revision indicated by the Master Drawing In-dex.

One out of four drawings, 2414 Revision 15, selected in the Quality Control Trailer did not correspond to the latest revisien indicated.by the Master Drawing Index.

One out of th,e four drawings, 2076 EF sheet I was not stamped controlled cooy in red, was on the distribution list for QC and was of the wrong revision.

B.

10 CFR 50, Criterion V states in part:

... procedures... shall in-clude appropriate quantitative... acceptance criteria for determin-ing that important activities have been satisfactorily accomplished."

The accepted Operational Quality Assurance Program (FSAR Section 17.2.17) states in part:

"... Tools... necessary for maintenance

... are calibrated... in accordance with station maintenance pro-cedures..."

2 9 MAR 1978 Contrary to the above, Preventive Maintenance Procedure M-48, Torque Wrench and Torque Wrench Testor Calibration, Revision 0, April 15, 1974 does not incorporate criteria for detenaining that the calibration of torque wrenches and the torque wrench testers have been satisfactorily accomplished. Calibration data for the torque tester (MEC-1) dated December, 1976, and torque wrench (MEC-

32) dated June,1977, indicate that these iten: had not met the tolercnces specified by the manufacturer and had been accepted as calibrated. The calibration data was reviewed by the inspector on February 23, 1978.
