APPENDIX A NOTICE OF VIOLATION South Carolina Electric and Gas Company License No. CPPR-94 V. C. Summer Based on the NRC inspection October 13 - November 9,1979, certain of your activities were apparently not conducted in full compliance with NRC require-ments as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by 10 CFR 50 Appendix B, Criterion V, as implemented by Section 4.6 of the Construction Quality Assurance Plan, documented procedures shall be followed for the preoperational test program.
Step 5.10 of procedure RC-01-H-1, Reactor Coolant System Cold Hydro Test requires that Reactor Coolant System heatup rate not exceed 50 F per hour.
Step 5.3 of procedure RC-01-H-1 requires 150*F maximum system temperature for the test as read on loop RTDs in service.
Contrary to the above, on November 2, 1979, during conduct of the a.
Reactor Coolant System Cold Hydro 'fest, per procedure RC-01-H-1, the Reactor Coolant System was heated up 55 F in one hour as indicated by loop RTDs.

Contrary to the above, on November 2, 1979, during conduct of the subject test, system temperature as read en the loop RTD indicated a temperature of 155 F.
