Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-373 Based on the results of the NRC inspection conducted on September 1-October 31, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

This item is an infraction.

10 CFR Part 50, Appendix B, Criterion XIV states, in part, " Measures shall be established for indicating the operating status of structures, systems, and components of the nuclear power plant, such as by tagging valves and switches, to prevent inadvertent operation." The Commonwealth Edison Company Quality Assurance Program for Nuclear Generating Stations, Section 14 states, in part, "The measures used to indicate status involve two phases. The first phase is pre-receipt controls.

The second phase consists of controls and identification of status onsite at the station.

This system will also provide for indicating the quality or operating status during construction and plant operations. Marking, tags, labels, forms, logbooks or other suitable means are used to identify operating status of parts and equipment to preclude inadvertent bypassing of the inspections and tests required prior to use. The operation status of station equipment is under control of the Shift Engineer."

Contrary to the above, the inspector's review of an occupational safety accident, which occurred on October 20, 1979, revealed that no procedural means have been established for the Shift Engineer to control the operating status of electrical power leads during station construction.
