4 APPENDIX A NOTICE OF VIOLATION Georgia Power Company License Nos. DPR-57 and HPF-5 E. I. Hatch Based on the NRC inspection September 5 through October 5, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspon-dence to you dated December 31, 1974.
As required by Technical Specification 6.8.1, written procedures shall be established and implemented for safety related activities. Hatch Nuclear Plant Procedure No. 314, Key Control, Paragraph B.3, states that keys for key operated switches on the control board are to be kept on the key board when not in use, and specifically states keys are not to be left in the switches. Hatch Nuclear Plant Procedure No. 17, Relief of Personnel, Paragraph D.1 states that the oncoming Plant Operator is to review entries made in the operator's log and initial each page.
It further requires that entries made the previous two shifts should be reviewed prior to assuming shift responsibilities.
Contrary to the above:
1.
On October 4, 1979, three keys associated with the high pressure coolant injection system and two keys associated with the Reactor Core Isolation Cooling System were being maintained in their respective switches on the control panel on Unit 2.
2.
The Unit 1 plant operator on October 4, 1979 and the Unit 2 plant operator on October 3, 1979, did not initial the previous two shift entries prior to assuming day shift responsibilities.
1his is an infraction. A similar item of the first example was brought to your attention in our letter dated August 3,1979.
