ENCLOSURE SEQUOYAH NUCLEAR PLANT UNITS 1 AND 2 PENN VENTILATOR QA PROGRAM NCR QEB 79-2 10 CFR 50.55(e)
FINAL REPORT Description of Deficiency During a TVA audit, a number of deficiencies were identified aaainst the Penn Ventilator Company QA program.
The deficiencies indicated that Penn Ventilator was not implementing the QA manual that had been submitted to TVA and approved for use.
Safety implications Statement Inspection of tha subject ventilators has established that the ventilators, as presently designed, do meet the necessary seismic and environmental qualifications for safety-related service.
Therefore, if the deficiency had gone uncorrected, it would not have adversely affected plant safety.
Corrective Action At the time the deficiency was discovered, sufficient documentation was not available to determine the acceptability of the ventilators. TVA first obtained and approved documentation for the seismic and environmental qualification of the ventilation components. A certificate of compliance from a seismic consultant was also obtained. TVA then requested detailed drawings of the subject ventilators from Penn Ventilator.
Upon receipt of these drawings, TVA reviewed them and found the seismic and environmental qualification documentation to be in compliance with the specification.
As a final step, TVA then initiated an inspection of the actual equipment to ensure that the equipment did correspond to the supplied documentation.
This inspection program was conducted and it demonstrated that, with regard to its safety function, the equipment was designed and manufactured in accordance with the approved drawings and documentation.
Similar problems have occurred on the Watts Bar and Bellefonte facilities with appropriate corrective action being described in separate reports.
