4 %45/79-27 50-446/79-26 Appendix A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on November 7, 1979, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Construction Permits No. CPPR-126 and 127 as indicated below:

Failure to Follow Procedures for Hoisting Safety-Related Components 10 CFR 50, Appendix B, Criterion V requires that activities affecting quality shall be accomplished in accordance with prescribed procedures, iratructions, or drawings.

Brown & Root Procedure CP-CPM-6.9, "Gcneral Piping and Inspection,"

paragraph 3.6 requires that vendor recommendations shall be reviewed by engineering and noted on installation procedures and that hoisting loads imposed on the building shell be reviewed by engineering prior to making hoists in excess of 2000 pounds.

Contrary to the above:

On November 7,1979, the NRC Resident Reactor Inspector (RRI) observed craft personnel preparing to hoist a large Class 1 motor operated valve (MOV) with what appeared to be inappropriate rigging.

Discussions with the craft personnel revealed that no instructions had been provided for hoisting the MOV.

It was determined that the MOV weighed in excess of 4000 pounds, but the building loads at the hoist point had not been evaluated by engineering.

This is an infraction.
