Dear Mr. O'Reilly:

Under the provisions of 10CFR21, NRC Region II was notified on October 23, 1979, that a potential significant deficiency or substantial safety hazard existed at North Anna lower Station, Unit 1.

In accordance with the reporting requirements of the above regulations, the following information is submitted:

Facility, activity and/or component affected: Control cable being in-stalled on a design change for degraded grid voltage, North Anna Power Station Unit No. 1.

Name of firm supplying the component, activity or service: Cerro/Rockbestos Corporation, Nicoll & Canner Street, New Haven, Connecticut 06504.

D.

Description of defect, deficiency, or failure to comply: A partially ex-posed wire was discovered in the area of an insulation splice, in a con-trol cable bundle (IENSHOC077) of twelve wires of size #14 AWG (NJK45-14).

Date of determination of reportability:

10/22/79.

F.

Similar components, activities, or services: no safety-related systems.

G.

Corrective action which has been, is being, or will be taken, the individu-al responsible and the length of time to complete the action: The faulty splice was removed and the cable was properly terminated.

H.

Other information: All like cable (spools 3187-150, 3187-151 and 3187-153) has been returned to the manufacturer for examination to determine if this q is a generic problem.
