Dear Mr. Eise nhut:
In response to your letter dated June 26, 1979 which was transmitted to us as an attacament to your letter dated September 21, 1979, a review was performed of sll occurrences at our facility to determine if any were similar in nature to the one described in your letter. This review indicated that no occurrences at our facility have resulted in or from a series of multiple equipment failures, but two occurrences with initiating events similar to the one described have occurred at our facility.
Each of these events resulted in an actuation of High Pressure Injection due to operator error but in each case the error was quickly recognized and correctly responded to by the operator. There was no power change or control rod movement resulting from these events and all equipment operated as designed. A review of these events indicated that the potential for problems that had not previously been considered does not exist.
Our review only considered occurrences in Modes one through four since these are the only Modes where our Plant Protective Systems are capable of producing a similar sequence of events to those of the one described.

Eisenhut Page Two November 9, 1979 Also a review was conducted of all applicable Engineered Safety System and Reactor Protective System Surveillance Procedures to determine whether appropriate cautioas are included and to ensure operators and supervisors are aware of the importance of avoiding challenges to the protective features of our facility.
This review indicated that our procedures provide adequate cautions to o pe ra to rs , technicians and supervisors sufficient to ensure that challenges to the protective features of our facility are avoided.
In addition to the above reviews that were conducted, the Nuclear Support Services Department is developing an NSSD procedure that will assure that multiple equipment failures in safety-related systems will be reviewed and analyzed to identify potential failure needs not previously considered, should they exist, that could lead to a significant reduction in the ability of safety systems to function as required. This procedure will be issued by January 15, 1980.
