APPENDIX A rf NOTICE OF VIOLATION Georgia Power Company License Nos. CPPR-108 Vogtle, Units 1 and 2 CPPR-109 Based on the NRC inspection July 24-26, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Criterion V of Appendi. B to 10 CFR 50, and as implemented by the Georgia Power Company PSAR Section 17.1.5,
" Activities affecting quality shall be prescribed by documented instructions, procedures c,r drawings...and shall be accomplish-d in accordance with these instructions, procedures, or drawings".
Calibration Instruction C-CI-09 requires daily calibration cf the balance scales in the soils / concrete laboratory by use of a set of test weights traceable to the National Bureau of Standards.
Contrary to the above, soils / concrete laboratory balance scales have not been calibrated.
In addition, test weights traceable to the National Bureau of Standards were not available on-site for scale calibration.
This is an infraction.
B.
As required by Criterion V of Appendix B to 10 CFR 50, and as implemented by the Georgia Power Company (GPC) PSAR Section 17.1.5,
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions, procedures, or drawings".
GPC Construction Procedure CD-T-02, Concrete Quality Control, paragraph V.E2.b(6) states, " Vibrators shall not be used for flowing concrete.... Spacing of vibrators shall be approximately 18 inches (9-inch radius from point of insertion)".
Contrary to the above, during placement of pour number A08D003D on July 25, 1979, vibrators were used for flowing concrete. The spacing of vibrators during concrete consolidation varied irom one to five feet.
This is an infraction applicable to CPPR-108. A similar item was brought to your attention in our letter dated December 12, 1978.
