Dear Mr. O'Reilly:
On September 28, 1979, a report was made under the provisions of 10CFR50.55(e) concerning a crack in the valve disc of MOV-2380.
In accordance with the reporting requirements of 10CFR21, the following information is submitted:
A.

Facility, activity, and/or component affected:
North Anna Power Station, Unit 2 Containment Isolation Valve MOV-2380 C.
Name of finn constructing the facility or supplying the component, activity or service:
Stone & Webster Engineering Corporation P. O. Box 2325 Boston, Massachusetts 02107 D.
Description of defect, deficiency, or failure to comply:
A crack was discovered on the disc of MOV-2380 while the disc seating surf ace was being refinished.
(The valve had failed to pass type C leak test.) A LP test was performed on the disc and the crack (linear indication) was confirmed.
In addition, another linear indication was discovered on the disc.

The disc of MOV-2380 is considered a pressure retaining part.
Further growth of the cracks in tM disc may render the valve incapable of performing its safety Qted containment isolation

Corrective action which has been, is being or will be taken, the Individual responsible and the length of time to complete the action:
Corrective action has been initiated. A replacement disc is being purchased and will be installed. We consider the linear indications in the valve disc to be an isolated incident. The supplier of this valve has informed us that no other defects involving these valve discs have been reported. Also we have no record of any such defects involving any other valve disc of this type.

Since no other actions will be required to correct the problem described in section D of this report, we consider this to be the final report under the provisions of 10CFR50.55(e)(3).
