APPENDIX A
, NOTICE OF VIOLATION Baltimore Gas and Electric Company Docket No. 50-318 Based on the results of the NRC inspection conducted on May 1-11, 1979 it appears that one of your activities was not conducted in full compliance with the conditions of your NRC ?acility License No. DPR-69 as indicated below. This item is an infraction.
Technical Specification 6.8.1 states, in part:
" Written procedures shall be established, implemented and maintained..."
Calvert Cliffs Instruction (CCI) 3008, "Calvert Cliffs Operating Manual" (CCOM), Revision 7. January 18, 1979, requires, in part, that valve lineup sheets be used; that an annotation be made in the coment column for valves which deviate from their desired position; and, that the Locked Valve Deviation Sheet be used when-ever a valve designated as " locked" on the valve lineup list is placed in other than its designated position.
OP-6, Pre-Startup Checkoff, Revision 10, October 25, 1978, Attach-ment 7, is, in part, a valve lineup checkoff sheet which specifies the normal operating position of certain valves in plant systems.
Contrary to the above, the CCOM and Attachment 7 to OP-6 were not properly implemented during a valve lineup checkoff accomplished by plant personnel on April 16, 1979 and an independent verification of this valve lineup by an NRC inspector on May 9 and 10, 1979, identified the following:
Two (2) valves were found to be in positions other than specified on the valve lineup sheet; Seventeen (17) valves required by the valve lineup checkoff sheet to be locked in position, although found to be in their correct position, were either not locked or were improperly locked.
One (1) valve was checked as being in its proper position during valve lineup checkoff accomplished on April 16, 1979, when, in fact, this valve was not accessible to the operator for observa-tion.
In each instance noted above, neither the valve lineup checkoff sheet nor the Locked Valve Deviation Sheet were annotated to reflect the discrepancies.
