NOTICE OF VIOLATIO'l Basec cr the result of an flRC inspection conducted during the period of July 8 -
Augus: 3,1979, it appears that certain of your activities were not con 61cted in full ccmoliance with flRC regulations and the conditions of your license, DPR-51, as incicated below:
A::endix A, Sections 1, 2 and 4 of 10 CFR 55, as im,olemented by the Arkansas fluclear One Requalification Program requires in part that:
A schedule be provided for requalification training; Lectures will be given to licensed operators in those areas of the annual examinaticr: in which the individuals attained a score of less than 80%;
Annual performance evaluations will be acccmplished.
Contrary to the above:
A schedule was not established; Three lectures were not given to licensed operators in those areas of the annual examination on which a score of less than 80% was attained; One licensed operator had not received an annual perfomance evaluation.
The above constitutes an item of noncompliance at the the infraction level.
