APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No. 50-245 Based on the results of an NRC inspection conducted on April 30 through May 4,1979 it appears that one of your activities was not conducted in full compliance with NRC regulations as indicated below.
Item A is an infraction.

10 CFR 20.201(b) requires each licensee to make or cause to be made such surveys as may be necessary to comply with the regulatory requirements specified in 10 CFR Part 20.
Contrary to the above requirement, surveys adequate to assure compliance with 10 CFR 20.101 were not performed in the vicinity of the drywell head on the refueling floor of the reactor building on May 2, 1979.
A beta radiation field with exposure rates as high as 100 mrad /hr at three inches from the head bolts remained undetected.
Dosimetry devices in use by personnel on the refuel floor would not record exposures to beta radiation and no restrictions were in effect to prevent access to the beta field surrounding the head bolts.
